摘 要: 目的 探讨Fas及其配体 (FasL)蛋白表达与胸腺瘤合并重症肌无力的关系。方法利用免疫组织化学染色方法,分析各期胸腺瘤伴有和不伴有重症肌无力患者胸腺组织内Fas及FasL蛋白表达程度,利用显微图像分析系统对结果进行量化统计分析。结果 各期胸腺瘤伴有重症肌无力者胸腺组织内Fas及FasL蛋白表达(Ⅳ期分别为 1 75% ±0 85%和 4 96% ±2 01% )明显低于不伴有重症肌无力者(Ⅳ期Fas及FasL分别为 25 80%±13 43%和 25 75%±13 13% ),差异有统计学意义(P<0 01)。结论 胸腺瘤伴有重症肌无力者,胸腺内T淋巴细胞凋亡能力下降,导致自身反应性T淋巴细胞克隆清除障碍从而引发重症肌无力等自身免疫性疾病。Objective To explore the significance of the apoptosis of lymph cell in the happenings of myasthenia gravis.Methods Thymus tissues of 133 patients with myasthenia gravis were studied. Using immunohistochemistry, we analyzed the expressive level of Fas/FasL protein in diverse stages of thymoma tissues with or without myasthenia gravis (134 cases), then quantitatively statistically analyzed the results with micro photo analysis system.Results The expressive level of Fas / FasL protein [including the positive cell area (1.75%±0.85% and 4.96%±2.01% respectively in stage Ⅳ) and color level] in diverse stage thymoma tissues with myasthenia gravis was obviously lower than that in thymoma without myasthenia gravis (Fas and FasL protein positive cell area were 25.80%±13.43% and 25.75%±13.13% respectively in stage Ⅳ), and the difference was significant (t test P<0.01).Conclusion Our study showed that the apoptosis ability of thymus tissue cell (lymph cell) might be decreased in the cases of thymoma with myasthenia gravis and then led to autoimmune lymphocyte clone clearance disorder, dysfunction of immune system and maybe become a main reason of myasthenia gravis.
