摘  要:目的 测定不同剂量组利培酮的有效代谢产物 9 羟利培酮的血浆浓度 ,探讨其临床意义。方法 对 5 6例精神分裂症患者应用利培酮 3、4、5mg ,第 4周末采用HPLC测定利培酮有效代谢产物 9 羟利培酮的血浆浓度。治疗前及治疗后第 4周进行阴阳性症状量表 (PANSS)的评定。结果 不同性别的口服利培酮剂量及 9 羟利培酮的血浆浓度存在显著差异。3、4、5mg组 9 羟利培酮血药浓度存在显著差异 ,利培酮剂量与 9 羟利培酮血浆浓度呈极显著性相关。 4周末的 9 羟利培酮血浆浓度与PANSS总分的减分率无明显相关而与阳性分量表分的减分率有显著性相关。结论  9 羟利培酮的血浆浓度与口服利培酮剂量有关 ,女性口服利培酮剂量及 9 羟利培酮的血浆浓度低于男性 。Objective To observe the relationship among oral dose, pla sm a drug level and the clinical significance of risperidone. Methods The study was conducted in 56 patients with schizophrenia. They were divid ed to receive 3 mg, 4 mg or 5 mg risperidone for 4 weeks. The plasma drug levels of 9-hydroxyrisperidone were measured by HPLC method at the end of Week 4. The PANSS were evaluated at Week 1 and 4. Results The difference of oral dose of risperidone and plasma levels of 9-hydroxyrisperidone were sig nificant between male and female patients. The plasma levels of 9-hydroxyrisper idone were significantly different among the 3 mg, 4 mg and 5 mg group. The oral dose of risperidone was significant positive correlated with the plasma levels of 9-hydroxyrisperidone. The plasma level of 9-hydroxyrisperidone was not corr elation with the decreased rate in PANSS total scores but was significant positi ve correlation with the decreased rate in positive scores. Conclusions The oral dose of risperidone was correlated with the plasma level of 9-hydroxyrisperidone. The oral dose of risperidone and the plasma level of 9- hydroxyrisperidone were lower in female patients than that in male patients so r isperidone can improve the patient′s positive symptoms firstly.
