斜视性弱视多焦VEP与多焦ERG的对比研究

摘　　要：目的　对比研究斜视性弱视患者多焦视觉诱发电位 (multifocalVEP)及多焦视网膜电图 (mERG)的特征性变化 ,探讨弱视发病的可能机制。方法　采用VERISScienceTM4 2多焦电生理系统记录并比较正常组 30例、斜视性弱视患者 2 0例各眼mVEP及mERG。结果　弱视眼不同视网膜区域mVEP、mERG的特征峰反应振幅密度都较对侧眼和正常眼明显降低 ,mVEP的特征峰潜时延长。弱视眼mVEP、mERG波形异常程度随离心度增加而减小 ,mVEP波形异常程度大于mERG ,且与弱视眼的视力异常程度有相关性。结论　斜视性弱视患者的mVEP和mERG具有明显的特征性改变 ,表明弱视患者的视网膜、视皮层都存在明显损害。Objective To compare the characteristic changes of multifocal VEP and multifocal ERG in strabismic amblyopia,to study the possible mechanism of amblyopia.Methods The multifocal pattern VEP and multifocal flash ERG of both eyes of 20 esotropic amblyopes and 30 normal subjects were recorded.Results In different retinal areas the N 1 P amplitudes of mVEP and mERG of the amblyopic eyes were reduced compared with the fellow eyes and the normal eyes.In amblyopic eyes the latencies of mVEP were markedly prolonged while no changes of the mERG latencies were found in amblyopic eyes.The abnormality of mVEP and mERG waveform in amblyopia decreased with the eccentricity increased,and the abnormality of mVEP is larger than that of mERG.The abnormalities of mVEP were correlated with the damage of visual acuity of the amblyopic eyes in all six eccentricites.Conclusions The results that mVEP and mERG of amblyopia showed characteristic changes suggest the existence of impairment of retina and visual cortex in amblyopia.
