摘 要:目的 探讨口腔白斑、扁平苔藓的癌变机制。方法 通过原位末端转移酶标记法 ,观察分析 10例正常口腔黏膜上皮 ,18例扁平苔藓 ,2 3例白斑 ,2 2例鳞癌上皮组织凋亡状况。结果 除单纯上皮增生 ,非糜烂型扁平苔藓外 ,上皮 (轻、中、重 )异常增生 ,鳞癌及糜烂型扁平苔藓的凋亡指数均高于正常 ,差异有显著性。从上皮异常增生到鳞癌凋亡指数逐渐增高 ,糜烂型扁平苔藓的凋亡指数低于鳞癌 ,差异有显著性。结论 细胞凋亡参与白斑癌变的过程 ,在病变不同阶段作用不同。Objective To investigate the pathogenesis of transformation of oral leukoplakia (LK) and oral lichen planus (LP) of oral squamous cell carcinoma (SCC). Methods TdT-mediated-dUTP nick end labeling (TUNEL) was used to observe the apoptosis of epithelial cells in 10 cases of normal oral mucosa,18 cases of LP, 23 cases of LK and 22 cases of SCC. Results The number of epithelial apoptosis in (mild, moderate, severe) dysplasia, SCC, and erosive LP was significantly larger than that in normal oral mucosa. With the dysplasia increasing, the number of apoptosis increased gradually. The number of apoptosis in erosive LP was significantly smaller than that in SCC. Conclusion Apoptosis may play a key role in transformation of LK into SCC and has various roles in different stages of cancerous transformation. The pathogenesis of LP may be associated with apoptosis increasing of epithelial cell, which was induced by cell mediated immunity.
