Discam彩色数字立体成像仪视乳头参数与青光眼视野损害的相关性研究

摘　　要：目的:研究Discam彩色视乳头数字立体成像仪测量的视乳头参数与开角型青光眼视野损害的相关性,探讨Discam彩色视乳头数字立体成像仪在开角型青光眼诊断中的应用价值。方法:采用Discam彩色视乳头立体成像仪在ScreenVu立体观察镜下确定31例(58眼)开角型青光眼及29例(58眼)可疑青光眼患者垂直杯盘比(verticalcup-discratio,V-CDR),水平杯盘比(horizontalcup-discratio,H-CDR),杯盘面积比(areacup-discratio,A-CDR)及沿盘面积比(arearim-discratio,A-RDR)。采用Humphrey自动电脑视野分析仪(30-2程序)对上述患者进行视野检测,研究上述视乳头参数与视野损害分组及视野指数视野平均缺损值(meandefect,MD)、视野模式标准差值(patternstandarddeviation,PSD)的相关性。结果:对116眼开角型青光眼及可疑青光眼患者研究发现,青光眼组上述视乳头参数与MD(P<0.000)、PSD(P<0.000)显著相关。而可疑青光眼组上述视乳头参数与MD、PSD无显著相关性。正常视野组及异常视野组视乳头参数的均值比较有显著统计学意义。结论:开角型青光眼患者Discam彩色数字立体成像仪视乳头参数与视野损害显著相关,Discam彩色数字立体成像系统可对视乳头进行定量描述并可用于评价开角型青光眼的视乳头损害。AIM: To determine the correlation between the optic nerve head (ONH) paramete rs of Discam colour digital stereoscopic camera and visual field damage in patie nts with open angle glaucoma, and to study the value of Discam colour digital st ereoscopic camera in the diagnosis open angle glaucoma. · METHODS: ONHs of open angle glaucoma patients (31 cases,58 eyes) and suspicio us glaucoma patients (29 cases,58 eyes)were photographed using Discam (Marcher, Ltd, U.K). Vertical cup-disc ratio (V-CDR), horizontal cup-disc ratio (H-CDR), area cup-disc ratio (A-CDR), area rim-disc ratio(A-RDR)were recorded in a screen Vu stereoscope. A system of the humphrey visual field (HVF) analyzer (program 3 0-2) was used to obtain the mean defect (MD) and pattern standard deviation (PSD ) values. HVF was classified as normal and abnormal. The correlations between Di scam ONH parameters and the visual field damage exponents were evaluated. · RESULTS: In the 116 eyes of open angle glaucoma and suspicious glaucoma, the V-CDR, H-CDR, A-CDR and A-RDR were significantly correlated with MD and PSD (P < 0.000) in glaucoma group. No significant difference existed in suspicious glauc oma patients. Eyes of abnormal HVF had significantly different ONH parameters me asured by Discam in comparison with eyes normal HVF. · CONCLUSION: A significant correlation was found between the Discam ONH parame ters and the degree of visual field damage in patients with open angle glaucoma. The Discam ONH imaging system provides a quantitative description of the ONH th at may be useful in assessing ONH damage in open angle glaucoma patient. ·
