P型空肠袢代胃术在胃癌全胃切除术中的应用

摘　　要：目的　探讨P型空肠袢代胃术在胃癌全胃切除术中应用的合理性。方法　38例胃癌病人经腹全胃切除 后,行食管-P型空肠袢吻合,空肠-空肠Roux Y吻合,术后加强营养支持治疗。结果　本组无手术死亡病例, 无食管空肠吻合口瘘等严重并发症发生,具有创伤小,恢复快,并发症少等特点。结论　经腹胃癌全胃切除P形空 肠代胃术是一种安全、实用、疗效满意的手术方法,术后加强营养支持治疗,有益于病人顺利康复。Objective To explore the rationality of application of P-shaped Roux-Y jejunal interposition in total gastrectomy for gastric carcinoma.Methods After total gastrectomy through celiotomy in thirty-eight cases of gastric carcinoma patients,P-shaped jejuno-esophagostomy and Roux-y jejuno-jejunostomy were used in operation,continuing with nutrition and sustaining treatment thereafter.Results There was neither death nor sever complications such as anastomotic leakage.And rapid recuperation.Conclusion Total gastrectomy through celiotomy with P-shaped jejuno-esophagostomy is an operation in terms of safety,satisfaction and practicality.It is in favor of patients with nutrition and sustaining treatment after operation.
