Survivin与高危型人乳头瘤病毒在宫颈癌及癌前病变中的表达和意义

摘　　要：目的:研究宫颈癌及癌前病变中凋亡抑制因子Survivin的表达及高危型HPV的感染情况,探讨两者在宫颈癌发病机制中的作用。方法:研究对象为宫颈上皮内瘤样病变(CIN)Ⅰ级2 3例、CINⅡ～Ⅲ级2 2例及宫颈癌31例,另取10例正常宫颈作对照。采用免疫组化SP法检测Sur vivin的表达;以touch downPCR技术检测高危型HPV(HPV16 ,18,31,33,5 8等)的感染率。结果:①从正常宫颈→CIN→宫颈癌,Survivin及高危型HPV表达均逐渐增强(P <0 .0 5 )。②低分化宫颈癌Survivin阳性表达显著高于高、中分化宫颈癌(P <0 .0 5 ) ,而与年龄、临床分期及组织学分型无关(P >0 .0 5 ) ;高危型HPV感染与宫颈癌年龄、临床分期、组织学分级及组织学分型均无关(P>0 .0 5 )。③Survivin与高危型HPV在宫颈癌中的表达呈正相关(P <0 .0 5 )。结论:Survivin表达及高危型HPV感染可能与宫颈癌的发生、发展密切相关。两者在宫颈癌的发病机制中可能起着协同作用。Objective:To in vesti gate the expressions of Survivin and high-r isk human papillomavirus in cervical carci noma and premalignant lesion ,and expl ore their role in the pathogenesis of cervi cal carcinoma. Methods:In 76 patients of cer vical intraepithe lial neoplasia and cervical carcinoma were enrolled, including 23 CINⅠ, 22 CINⅡ～Ⅲ and 31 in vasive carcinoma, The expressions of Surv ivin were examined by immuno-histochem ical assay; and the infection rates of high -risk HPV including HPV16, 18, 31, 33, 52, 58 were measured by touch-down p olymerase chain reaction(PCR), 10 normal cervical specimens were taken as control. Results:①Expressions of Survi vin and high-risk HPV tended to increase along with tumor pro gression(P<0.05); ②The differe nce of Survivin expression in different hi stolo gical grade of invasive carcinoma was ssignificant(P< 0.05 ),but th e expression of Survivin in different pa tients age,clinical stage and histologic al clas sification had no statistical differenc e(P>0.05); high-risk HPV expres sion statistically had nothing to do with a ge,clinical stag e,histological grade and histological clas sfication (P>0.05);③Positive correlatio n between the express ions of Survivin and high-risk HPV was obse rved in cervical carcinoma. Conclusions:The data suggeste d that Survivin might play an important role in the occurre nce and develo pment of cervical carcinoma together with h igh-risk HPV.
