周剂量草酸铂联合低剂量氟脲嘧啶治疗晚期高龄人胃癌的临床研究

摘　　要：目的 :探讨周剂量草酸铂 (L OHP)联合低剂量氟脲嘧啶 (两周疗法 )治疗 70岁以上晚期高龄胃癌的临床疗效和毒副反应。方法 :采用深静脉持续静脉输注 5 FU 2 5 0mg/m2 ·d ,持续 2周 ;静脉滴注L OHP 6 5mg/m2 ,每周 1次 ,连用 2周。每 3～ 4周重复 ,每例进行 2～ 4周期化疗后进行临床评价。结果 :全组 33例 ,总有效率为 4 8 5 %。其中 2 0例初治组有效率为 5 0 % ,有CR 2例 ;13例复治组有效率为 4 6 2 %。主要为Ⅰ、Ⅱ度白细胞下降和消化道反应及少数患者的Ⅰ度周围神经毒性。结论 :周剂量草酸铂 (L OHP)联合低剂量氟脲嘧啶 (2周疗法 )可能是治疗Objective:To evaluate the efficacy and toxicity of the treatment of advanced gastric cancer in the patients older than 70 years with weekly dosage oxaliplatin(L-OHP) and continuous low fluorouracil(5-FU)(two week therapy).Methods:33 patients received LD-5FU/L-OHP(L-OHP 65 mg/m 2/W,iv.gtt.2 hours,week 1,2;5-FU 250 mg/m 2/d,continuous central venous infusion,day 1～14),the second treatment cycle was given after 3～4 weeks.At last 2～4 cycles were given.Results:The overall response rate was 48.5%.The response rates of patients in primary treatment was 50%(2 CR) and that retum cases was 46.2%(P<0.05).The major toxic effect were myelosuppression(grade Ⅰ-Ⅱ),symptoms of gastrointestinal side-effects (grade Ⅰ-Ⅱ) and peripheral neuropathy toxicity (grade Ⅰ).Conclusion:Weekly dosage oxaliplatin(L-OHP) and continuous low dosage fluorouracil (5-FU)(two week therapy)has a high response rate with tolerable toxicity in the patients older than 70 years with advanced gastric cancer.
