热休克蛋白27在乳腺癌中的表达及与预后的关系

摘　　要：目的 :探讨热休克蛋白 2 7(Heatshockprotein2 7,HSP2 7)在乳腺癌中的表达 ,评估其在乳腺癌预后中所起的作用。方法 :用免疫组织化学方法检测 6 2例手术切除的乳腺癌组织中HSP2 7的表达 ,研究其与临床病理特征的关系及对预后的影响。结果 :①HSP2 7在乳腺癌组织中的阳性表达率为 4 1 9% (2 6 / 6 2 )。②HSP2 7表达同各临床病理特征之间的相关性不具有统计学意义。③Kaplan Meier生存分析表明 ,HSP2 7阴性表达组的无病生存期 (DFS)及总生存期 (OS)的生存率曲线均高于阳性表达组 ,但差异没有统计学意义。④Cox回归单因素分析显示腋窝淋巴结转移个数、组织学分级、肿瘤大小及TNM分期与DFS及OS明显相关 ,而年龄、HSP2 7阳性表达与DFS及OS的相关性不具有统计学意义 ;Cox回归多因素分析表明仅有腋窝淋巴结转移个数、组织学分级及肿瘤大小与DFS及OS明显相关 ,而TNM分期、HSP2 7阳性表达未进入Cox回归模型。结论 :HSP2 7在乳腺癌组织中有一定程度的表达 ,HSP2 7阳性表达与DFS或OS的缩短有一定相关性 ,但不具有统计学意义 。Objective:The purpose of this study was to examine expression level of heat shock protein 27(HSP27) and to investigate the relationship between its expression and prognosis in primary breast cancer patients. Methods:An immunohistochemistry study was performed to measure expression level of HSP27 in 62 primary breast cancer patients.HSP27 expression in the carcinomas was also studied with its relation to clinicopathologic parameters and prognosis.Results: ①The expression level of HSP27 in breast cancer tissue is 41.9%(26 in 62 ).②There were no associations between HSP27 expression and clinicopathological parameters,such as tumor size,axillary metastatic lymph node numbers,histological grades and menstrual status.③Kaplan-Meier survival analysis showed that DFS(disease free survival)and OS (overall survival)curve of the group which expressed HSP27 positively were higher than the one expressed negatively,but the difference didn't have statistical significance.④Cox univariate regression analysis showed that DFS and OS markedly correlated with axillary metastatic lymph node numbers,histological grades,tumor sizes and TNM stages.HSP27 positive expression and ages didn't correlate with DFS and OS.However,through Cox multivariate regression analysis,we found that only metastatic lymph node numbers,histological grades and tumor sizes showed significant associations with DFS and OS.HSP27 positive expression and TNM stages didn't enter into Cox regression model.Conclusion:HSP27 showed some degree of expression in breast cancer tissues.HSP27 positivity associated with the shortening of DFS or OS,but it didn't have statistical significance,so HSP27 expression was not of independent prognostic importance for prognosis in breast cancer patients.
