摘  要:目的 :探讨上颌前方牵引联合上腭快速扩大对伴有上颌发育不足的骨性安氏Ⅲ类错牙合患者的颅面结构和上气道间隙的影响 ,以及颅面形态改变与上气道间隙改变之间的关系。方法 :研究对象共 2 6人 ,为骨性安氏Ⅲ类错牙合伴有上颌骨深度发育不足 ,无颌骨高度发育不调。根据矫治前X线头颅定位侧位片上颈椎发育程度判断 ,均为生长发育期患者。行上腭快速扩大后 ,开始上颌前方牵引治疗。结果 :在多因素分析中 ,3个上气道头影测量变量 (SPPS ,MPS ,IPS)均能建立显著有效的回归方程。结论 :颅面形态改变与上气道间隙改变之间能建立有效的回归方程。前方牵引后 ,上颌向前生长及下颌的改变与上气道间隙改变有显著的联系 ,说明上颌前方牵引能影响上气道间隙。Objective:This study was conducted to examine the effect of treatment with rapid maxillary expansion (RME) and maxillary protraction appliance on the development of the craniofacial structures and upper-airway dimensions in 26 subjects exhibiting a Skeletal Class III malocclusion with a retruded maxilla, and to evaluate the relationship between changes in the craniofacial morphology and upper-airway dimensions. Methods: 26 subjects were skeletal Class III malocclusions with maxillary retrognathism,no skeletal height disharmony and treated using RME and facial mask. Results: In all 3 cephalometric variables of upper-airway dimension (SPPS, MPS, IPS), statistically useful regression equations could be established. Conclusion: A multiple-regression analysis revealed that maxillary growth and the change of mandible had a significant effect on the superior upper-airway dimension. These findings indicate that the superior upper-airway dimension can be altered during maxillary protraction.
