膀胱移行细胞癌HPV-16/18感染与p53基因改变的关系

摘　　要：目的　初步探讨膀胱移行细胞癌 (TCC)中HPV 16/ 18感染与p5 3基因改变的关系。方法　选择已确定为HPV 16/18感染的 18例膀胱TCC患者 (其中HPV 16/ 18E7片段阳性者 13例 ,HPV 16/ 18E7阴性者 5例 ) ,采用微卫星方法检测肿瘤组织内与 p5 3基因紧密连锁的TP5 3位点杂合性缺失 (LOH )情况 ,并采用免疫组织化学方法检测p5 3蛋白表达情况。 结果　 18例TCC组织中TP5 3位点杂合率为 83 .3 3 % (15 / 18) ,其LOH率达 46.67% (7/ 15 ) ;p5 3蛋白阳性率为 5 0 .0 0 % (9/ 18)。其中 ,HPV 16/ 18E7DNA阳性和 p5 3基因改变 [包括TP5 3位点LOH和 (或 )p5 3蛋白阳性 ]并存者 11例 ,占 61.11% (11/ 18) ;仅HPV 16/ 18E7DNA阳性或p5 3基因改变 [包括TP5 3位点LOH和 (或 ) p5 3蛋白阳性 ]者 5例 ,占 2 7.78% (5 / 18) ;两者均阴性者 2例 ,占11.11% (2 / 18)。结论　HPV 16/ 18感染可能通过 p5 3基因改变在TCC的发生发展过程中发挥作用 ,这种 p5 3基因改变可以是LOH和 (或 )蛋白异常表达。Objective To investigate the relationship between HPV-16/18 infection and the genetic abnormalities of p53 gene in transitional cell carcinoma(TCC).Methods 18 cases of TCC with documented HPV-16/18 infection(E7 segment positive:13 cases,E7 segment negative:5 cases).TP53 locus,which was closely linked with p53 gene was detected by microsatellite analysis and the expression of p53 protein was examimed by immunohistochemical staining in tumor tissue.Results The rate of TP53 locus was 83.33%(15/18),and the loss of heterozygosity(LOH) rate was 46.67%(7/15);the positive rate of p53 protein was 50.00%(9/18).The coexistence rate of HPV-16/18 infection and p53 genes alterations(including TP53 locus LOH and/or p53 protein positive) was 61.11%(11/18) in TCC.5(5/18,27.78%) cases were found HPV-16/18 E7 segment positive or p53 gene alterations.2(2/18, 11.11%) cases were nagetive for both HPV-16/18 E7 segment and p53 gene alterations.Conclusion The infection of HPV-16/18 may play a role in the carcinogenesis of TCC through the alterations of p53 gene,which include LOH and(or) abnormal expression of p53 protein.
