摘  要:目的 研究端粒酶逆转录酶 (hTRT)在膀胱移行细胞癌BTCC中的表达及其与肿瘤分级、分期、复发之间的关系。方法 采用原位核酸分子杂交法 ,对 5 2例BTCC和 10例正常膀胱中hTRT的表达情况进行检测 ,分析其在膀胱癌和非癌膀胱组织中的表达以及与肿瘤病理学分级、分期和患者复发情况的关系。结果  5 2例BTCC中hTRT表达均较正常膀胱黏膜增高 ,阳性率为 90 4 % (P <0 0 0 1) ;hTRT的表达随BTCC的病理分级、分期的增高而相应增高 ,Ⅰ、Ⅱ、Ⅲ级中表达率分别为 80 0 %、95 8%、10 0 % ,浅表性 (Tis T1 )、浸润性 (T2 T4 )中表达率分别为 87 0 %、93 1% ,初发BTCC、复发BTCC表达率为 87 5 %、95 0 % (P<0 0 5 ) ;结论 人端粒酶逆转录酶 (hTRT)在BTCC中表达上调 ,提示hTRT可能通过激活端粒酶 ,使细胞无限增殖 ,对BTCC的发生发展起重要作用 ;hTRT随BTCC病理分级、分期增高而表达增高 ,并与肿瘤复发有关 ,hTRT有可能作为判断分级、分期 ,监测复发的指标 ;hTRT可能为膀胱癌基因治疗提供新的合理靶点。Objectives To study the expression of hTRT in BTCC; and its relationship between tissue grade, pathology stage, recurrence of bladder transitional cell carcinomas (BTCCs).Methods 52 bladder transitional cell carcinomas and 10 normal bladder mucosas were used to detect the expressions of hTRT by using in situ hybridization.Results The expression of hTRT in 52 BTCCs is higher than that in 10 normal bladder tissues (P<0.05). The positive rate is 90.4%. The expression level of hTRT is elevated as the grade and stage increased in BTCC. The positive rates in grade Ⅰ、Ⅱ、Ⅲ are 80.0%、95.8%、100%; in stage of Tis-T 1、T 2~T 4 are 87.0%、93.1%; and in primary、recurrent cases are 87.5%、95.0%, respectively. All of the above data are statistically significant at a level of P<0.05.Conclusions The up-regulated expression of hTRT in BTCC suggests that, by activating telomerase, hTRT plays an important role in cell proliferation and tumor occurrence and progress. The expressions of hTRT are elevated as tumor grade and stage increased. The study also shows that hTRT is associated with tumor recurrence. Therefore, hTRT can be used as effective indicator to identify stage, grade and recurrence. Elevated expression of hTRT can play a synergic role on occurrence and progress of BTCC. hTRT can provide new appropriate target for gene therapy to BTCC.
