川芎嗪对胸部肿瘤放射治疗后肺损伤的干预作用

摘　　要：目的　观察川芎嗪对胸部肿瘤患者放射治疗的肺保护作用及血浆转化生长因子 β1(TGF β1)表达的影响。 方法　将胸部肿瘤放射治疗者 4 0例随机均分为川芎嗪组和对照组。川芎嗪组在放射治疗期间服用川芎嗪 15 0mg/次 ,3次 /d。放射治疗前后测定血浆TGF β1,放射治疗结束后3、6个月随访观察临床症状、高分辨率CT和肺功能。结果　川芎嗪组放射治疗前血浆TGF β1为( 4 .4 5± 1.76 )ng/ml,放射治疗后为 ( 5 .79± 2 .88)ng/ml;对照组放射治疗前为 ( 4 .4 8± 2 .4 0 )ng/ml,放射治疗后为 ( 11.70± 5 .4 8)ng/ml,较川芎嗪组明显升高 (t=4 .92 ,P <0 .0 0 1)。放射治疗后 6个月川芎嗪组CO弥散量明显下降者 (下降幅度 >2 5 % )占 31.6 % ( 6 / 19) ,对照组占 6 3.2 % ( 12 / 19) ( χ2 =3.89,P<0 .0 5 )。川芎嗪组急性放射性肺炎 2级 1例 ,晚期放射性肺损伤 1级 2 0例 ;对照组急性放射性肺炎3级 1例 ,晚期放射性肺损伤 1级 19例 ,3级 1例。结论　川芎嗪能抑制放射治疗后血浆TGF β1的过度表达 。Objective To observe the effect of Ligustrazine on radioprotection of lung and expression of plasma transforming growth factor beta-1 (TGF-β1) in patients with lung radiotherapy. Methods Forty lung irradiated patients were randomized into control group (20 patients) and Ligustrazine group (20 patients) with Ligustrazine 150?mg, po, tid, during radiotherapy. Clinical symptoms, lung function, and high-resolution computed tomography of thorax were evaluated before and at 3 and 6 months after radiotherapy. Plasma TGF-β1 was examined by ELISA before and at the end of radiotherapy. Results At the end of radiotherapy, the level of plasma TGF-β1 in control group was much higher than in Ligustrazine group (11.70±5.48?ng/ml vs 5.79±2.88?ng/ml, t= 4.92, P < 0.001). Six months after radiotherapy, patients in the Ligustrazine group whose CO diffusion capacity of Lung (DLCO) declined by more than 25% were less than those in the control group (31.6% vs 63.2%,χ 2 = 3.89, P < 0.05). In Ligustrazine group, 1 patient suffered from grade 2 acute radiation pneumonitis, 20 patients grade 1 late radiation lung injury, as compared with those in control group in which there was 1 patient with grade 3 acute radiation pneumonitis, 19 patients grade 1 and 1 patient grade 3 late radiation lung injury. Conclusions Ligustrazine can inhibit overexpression of plasma TGF-β1 after radiotherapy, and may reduce the severity of dysfunction of radiation lung injury.
