摘 要:目的 探讨MR弥散加权成像 (DWI)的扫描技术参数的选择在急性脑梗塞诊断中的应用价值。方法 对 3 6例急性脑梗塞患者进行头颅常规MRl扫描和不同扫描技术参数的DWI扫描。结果 全部急性脑梗塞在DWI上均表现为高信号。在DWI上 ,弥散全方向与单方向对于显示脑灰质的梗塞灶效果一样 ,但弥散全方向比单方向显示脑白质梗塞灶清楚 ;b值越高 ,弥散效果越好 ,显示梗塞灶越清楚 ;12 8× 2 5 6的采集矩阵比 12 8× 12 8和 12 8× 64的图像效果好。结论 DWI对急性脑梗塞的诊断具有很高的敏感性和准确性 ,其弥散方向、b值和采集矩阵的选择是影响弥散效果的关键因素。Objective To explore the appled value of selecting the scanning parameters of diffusion-weighted imaging(DWI) in detecting acute cerebral infarction.Methods 36 patients with acute cerebral infarction were imaged with conventional MRI and DWI using the different scanning parameters. Results The lesions of all acute cerebral infarction showed hyperintensity on DWI. The infarct lesions of gray matter in brain were showed on all direction of diffusion is the same as that on single direction of diffusion, but the infarct lesions of white matter in brain were showed more clearly on all direction of diffusion than that on single direction of diffusion. The higher b value, the better effect of diffusion, and the more clearly infarct lesion could be showed. The effect of DWI with a matrix of 128×256 was better than that with a matrix of 128×128 or 128×64.Conclusion DWI is sensitive and accurate in detecting acute cerebral infarction. Direction of diffusion, b value and matrix are the key factors of the effect of diffusion .
