人体眼眶包虫病13例临床分析

摘　　要：作者综合分析报告了人体眼眶包虫病13例。其占新疆地区全身包虫囊肿4276例的0.3％。结合临床实践系统地阐述了其流行病学、临床特点、诊断技术和治疗原则。并总结提出了诊断、治疗眼眶包虫病的要点与措施。The authors analyzed 13 cases of human orbital echinococcosis. It is 0.3％ out of 4276 cases of human echinococcosis in Xinjiang. 5 cases were men, 8 cases were women, the youngest 3 years old and oldest 49 years in age. The majority are farmers herdsman and their children. There were exophthalmosis and impairment of vision but different in degree. The longest history was 3 years and shortest was one month, average was 6 months. In 10 cases the cysts located in the superior part of the orbit and 3 cases in inferior. All were in the muscular corne between orbital bones. The diagnostic criteria of orbital echinococcosis are: (1) The patient come from an epidemic area. (2) Clinical appearence with occupied cyst in orbit. (3) Cystic change was seen in the X-ray, ultrasonic and CT examination. (4) Positive reaction of serum immunological tests(IHA, ICEP, ID). Early treatment by operation to completely extract the cyst for protecting vision, avoiding flow ous of fluid from the cyst to produce secondary infection or hypersensitive reaction of the tissue. So it is advocated: (1) Localize the position exactly by ultrasonic or CT and take best route for extract operation. (2) Fine illumination for operation under direct observation with carefull technique to prevent cyst breaking. (3) May aspirate out of fluid inside of cyst and injecting in 10％ formalin or 3％ dioxide liquor at the beginning of operation, about 5～10 minutes later protoscoleces will be killed then performing extrac- tion operation. (4) Completely extracting the cyst and well suture the remaining space. (5) Giving antibiotics, corticosteroids or routinely i. v hypertonic substances postoperatively to decrease tissue reaction. All cases up today are well and no recurrence so far.
