术前放疗与根治术结合治疗子宫颈癌临床分析

摘　　要：应用手术方法治疗子宫颈癌110例,其中93例行子宫颈癌根治术。A组44例(Ⅰ_b期4例,Ⅱ_a期13例,Ⅱ_b期27例)采用术前放疗与根治术结合方法治疗,其中17例Ⅱ_b期患者行根治性放疗后成功地施行根治术。手术均于放疗后4～6周进行。B组49例(Ⅰ_a期1例,Ⅰ_b期35例,Ⅱ_a期13例)单独采用手术方法治疗。两组比较,手术经过均顺利,并发症发生率低,近期效果肯定。因之,对早期子宫颈癌,包括一部分Ⅱ_b期病例,放疗后再行根治术可以作为一种治疗选择;两种方法结合,放疗可以破坏有活力的瘤细胞,减少术中种植与扩散;使病灶缩小,为手术创造条件;手术可以诊断和切除放疗后残留病灶,减少复发,以期提高生存率。One hundred and ten patients with carcinoma of the cervix were treated with surgery since 1990 to 1992. On 93 cases of them radical hysterectomies and pelvic lymphadenectomies were performed. Of the 93 cases, 44 were treated by combined preoperative radiation and surgery(Group A, Ⅰ_b 4, Ⅱ_a 13 and Ⅱ_b 27). Among them, pre-operative radical radiotherapy were given in 17 cases with stage Ⅱ_b. And the radical surgical operations were performed within four to six postirradiation weeks in all cases. The remaining 49 cases were treated by radical surgery alone(Group B, Ⅰ_a 1, Ⅰ_b 35 and Ⅱ_a 13). In these series, the operations were uneventful with minor complications and definite therapeutic effects in both groups. Along with the reviewing of literatures, authors suggest that in early carcinoma of the cervix including some cases with stage Ⅱ_b the radical surgery may be a method of choice following irradiation. Its advantages are that irradiation destroys vital tumor cells thus operative implantation and spread may be diminished. The shrinkage of the tumor may give the way to surgery. While the surgery may establish the accurate diagnosis and concomitantly the residual lesions after radiation, so as to diminish the recurrence and raise the survival rate of this disease.
