外科治疗局部晚期非小细胞肺癌的疗效分析

摘　　要：目的 :分析有手术适应症的Ⅲb期非小细胞肺癌 (NSCLC)患者的外科治疗的疗效 ,寻找提高疗效的策略。方法 :1988年至 1997年 96例Ⅲb期NSCLC患者接受了外科手术治疗。根据是否接受术前新辅助化疗分为单纯手术组 ( 47例 )和术前新辅助化疗组 ( 49例 ) ,比较两组的手术性质及治疗结果。结果 :术前新辅助化疗组的肉眼下手术完全切除率 ( 46 9% )和 1、3、5年生存率 ( 42 5 %、3 4 7%、2 4 5 % )均明显高于单纯手术组 ( 2 3 4%和3 6 2 %、2 1 3 %、12 8% ,P <0 0 5 )。结论 :局部晚期非小细胞肺癌Ⅲb期患者术前接受新辅助化疗较单纯手术可提高外科治疗的疗效 。Objective: To analyze the curative effect of operable stage Ⅲb non-small cell lung cancer with surgical theatment and to investigate the better treatment strategy.Methods:From 1988 to 1997 96 patients with operable stage Ⅲb non-small cell lung cancer received surgical operation.They were divided into two groups according to treatment method:operation alone group(n=47) and preoperative neoadjuvant chemotherapy group (n=49).The operative quality and curative outcome were compared between these groups.Results:The gross complete resection rates and 1-,3-,5-years survival rates are signifiantiy higher in preperative neoadjuvant chemotherapy group(46.9% and 42.5%,34.7%,24.5%) than those of in operation alone group(23.4% and 36.2%,21.3%,12.8%,P<0.05)respectinely.Conclusion:Compared with operation along,preoperative neoadjuvant chemotherapy can obviously improve surgical curative effect in patients with operable locally advanced(stage Ⅲb)non- small cell lung cancer.
