胃粘膜病变与胃癌相关性初探

摘　　要：目的 :通过对免疫组化和粘液组化指标检测 ,分析各类型胃粘膜病变的增生和异型性情况 ,探讨其与胃癌发生之间关系。方法 :应用免疫组织化学和粘液组织化学方法对我院 2 0 0 1年至 2 0 0 3年间收治的 35 2例各类胃粘膜疾病患者胃镜活检标本的增殖细胞核抗原 (PCNA)、突变型肿瘤抑制因子(P53 )、癌胚抗原 (CEA)、硫酸粘液表达情况进行分析。结果 :①肠化腺上皮细胞中硫酸粘液成份及不典型增生程度与PCNA、P53 、CEA表达有关 ;②各类型胃粘膜病变的PCNA、P53 、CEA表达情况不同 ,呈现胃癌组〉不典型增生组〉肠化生组〉浅表性胃炎及萎缩性胃炎组 ,且PCNA、P53 、CEA趋势大致相同 ;③肠化生硫酸粘液阴性、阳性两组与不同程度不典型增生各组PCNA、P53 、CEA表达情况比较稍有不同。结论 :PCNA、P53 、CEA高表达、硫酸粘液阳性的肠化生和中、重度不典型增生与胃癌发生关系密切。OBJECTIVE :To study the proliferation and heterotype of different gastric mucosa diseases, and discuss the relationship between them and gastric cancer. Methods:By using immunohistochemistry and mucus-histochemistry , We determined the expressions of PCNA , P53, CEA, Sulphuric acid mucus in endoscopic biopsy specimens of 352 cases during 2 years from 2001 to 2003. Results:①There is correlation between Glandular epithelium cell of intestinal metaplasia with Sulphuric acid mucus,dysplasia degree and expression of PCNA , P53, CEA;②expressions of PCNA , P53, CEA in different gastric mucosa diseases are different. It showes a tendency: gastric cancer group>dysplasia group>intestinal metaplasia group>chronic superficial gastritis and chronic atrophic gastritis group. But The tendency is alike among expressions of PCNA , P53, CEA;③Expressions of PCNA , P53, CEA are a little difference between various dysplasias and intestinal metaplasia with Sulphuric acid mucus or without Sulphuric acid mucus.Conclusion: Gastric cancer is close relation to the high proliferation intestinal metaplasia with Sulphuric acid mucus and dysplasia that are high express of PCNA, P53, CEA.
