腭裂修复术后语言障碍的手术矫治

摘　　要：目的:为改善腭裂修复术后语言障碍患者的说话功能。方法:对6例腭裂术后语言功能极差患者,根据腭咽闭合功能障碍的程度,分别采用了咽后嵴成形和咽后瓣移植等二种方法,进行二期手术矫治。结果:6例矫治术前说话他人难以听懂或完全听不懂定为差、劣水平的语言状态,术后达到说话他人已大部分能听懂的良好水平。结论:腭咽闭合不良是腭裂修复术后语言障碍的主要原因,应在腭裂修复的同时行一期矫治。一期未能矫治者,应积极进行二期手术矫治,尽可能地改善其语言功能。Objective: To improve the speech function of the patients with speech and language disorder at post repair operation of deft palate. Method: Of the 6 patients, 2 cases had mucous menbrane- ridge form at retropharyngeal wall, 4 cases had mucous menbrane- flap graft at between soft palate and retropharyngeal wail. Results: The faculty of speech has much improvement, which was from indistinct to dear in all six patients. Conclusion: Connot shut to the palate - pharynx is major reason that leading to speech and language disorder. It was necessary that secondary operative rectification on the patients with speech and language disorder at post repair operation of the cleft palate for to improve speech function of the patients.
