硝普钠对大鼠重症胰腺炎血浆一氧化氮影响的实验研究

摘  要:目的:观察硝普钠对大鼠重症胰腺炎的治疗作用.方法:本实验采用牛磺胆酸钠塑胰胆管注射诱发大鼠急性重症胰腺炎(ANP),动态观测ANP模型制作后(4、12、24、48h)血浆NO、SOD、血浆淀粉酶等指标的变化及硝普钠(SNP)对其影响,并取胰腺组织进行病理切片观察.观察结果:ANP组在4h后血浆NO、SOD即明显下降(P<0.05),并持续24h以上血浆淀粉酶及胰腺系数(PI)明显升高(P<0.05);病理可见胰腺腺泡高度肿胀,间质内大量炎细胞浸润及红细胞渗出,伴有较多坏死灶.SNP治疗组血浆NO、SOD明显高于非治疗组(P<0.05);血浆淀粉酶含水量明显低于非治疗组(P<0.01);胰腺系数明显低于非治疗组(P<0.05);病理可见胰腺腺泡呈现轻、中度肿胀,局灶腺泡坏死,间质少量炎性细胞渗出.结论;急性重症胰腺炎时期即出现胰腺微循环障碍,胰腺组织缺血缺氧,导致NO含量下降及脂质过氧化反应增强,继而引起一系列继发性损害.本实验结果亦提示:NO供体药物硝普钢能够改善胰腺微循环,减轻胰腺水肿,减少胰腺外分泌,抑制胰腺组织的脂质过氧化反应.因此可以通过NO供体药物试验性治疗急性重症胰腺炎、以减轻继发性操作,为临床急性重症胰腺炎的治疗提供一新的途径.Objective The experiment is to observe the efttct of SNP treatment on ANP ofras.Method (Using ANP model in rats,we dynamically observed the changes of the levels of No, SOD and amyIase in plasma at points(4.12.24,48.hours),as well ea the of fect of sodium Nitroprusside(SNP) on them in the study.While we also observed the pathological alterations ofpancreatic tissues.Result The results shotved that the NO and SOD leveli in pIasma tvere reduced(p < 0.05)and lasted more than 24 hours in modeI control group;the amylase activities in plasma and Pl were increased(p< 0.05);while eddma PMN infiltration .RBC and necrosis of lipid cell could be seen under light microscope The levels of NO and SOD in plasma in SNP treatment group tvere higher(p<0.05),the levels of amylase in plasma were lower(p<0.01),and the levels of PI were less(p < 0.05).when comparing with the model control group.Concfusions The results suggest that the pancreatie microirculation was obstacled after ANP,anoxia ghd ischemia were occurred in tissues.this can result in lipid peroxidation enhancement witch caused a Serises of damage The results ofour study also indicate that SNP.a sort of damage. NO donordrugs can improve pereatic mimpirculation and edema, reduce pancreatic exocrinosity,inhibit lipid perxidation and reduce the mortality rae,There fore,No donor drugs can accured ANP in erpeximent and reduce damage.Mean While,it can provide a new method of treatment of clinic ANP
