层流室在恶性血液病大剂量化疗中防感染作用

摘　　要：目的：为观察层流室（ＬＡＦＲ）在治疗恶性血液病中防感染作用。方法：我们回顾性分析了经骨髓移植或外周血干细胞移植的１２例白血病（ＢＭＴ组），１０２例大剂量化疗的恶性血液病（ＨＤＣ组），１５２例经标准化疗的恶性血液病（ＳＣ组）三组资料。前两组在层流室进行治疗，后一组在开放病区。结果：空气标本微生物培养阳性率在层流室中为０，与其它房间相比（０．０４８～０．０９５）有显著差异（Ｐ＜０．００５）。在ＢＭＴ、ＨＤＣ、ＳＣ３组中，平均白细胞数下降至０、０．９８×１０９／Ｌ、１．５１×１０９／Ｌ，而感染率分别为０．７５０、０．２３５、０．２７６。Objective: To determine the role of LAFR on protect hematological malignancies from infections.Methods: We retrospectively analysed the follow three groups.The 12 cases of leukemias underwent bone marrow transplant or peripheral blood stem cell transplant(BMT).The 102 cases of hematological malignancies were treated with high-dose chemotherapy (HDC).The two groups were treated in the LAFR.The 152 cases of hematological malignancies were treated with standard chemotherapy (SC)in the open ward.Results: The positive rate of microorganism culture of air sample was 0 in LAFR,and had significant difference (P<0.005)as compared with the other rooms (0.048-0.095)in the closed wards for BMT.In our series,the mean WBC in BMT,HDC,SC group decreased 0,0.98×10 9,1,51×109/L,the incidence was 0.750,0.235,0.276.Conclusion: LAFR has a important role of protect patients from infections in high-dose chemotherapy.
