老年非胰岛素依赖性糖尿病胰腺CT检查与临床分析

摘　　要：24例老年非胰岛素依赖性糖尿病(NIDDM)进行胰腺 CT 检查,并设对照组107例,对胰腺头、体、尾3部位作了前后径测定及形态结构上进行了比较观察。结果:病人组的胰腺3部位均明显小于对照组。根据测定值低于正常范围低值(均数-1.96标准差)为缩小,则头、体、尾3部位缩小数分别为5、2及8例。老年 NIDDM 者胰腺轮廓均有切迹,且极大部分(94～100%)结构不均匀。作者将前后径中有一项低于正常最低值者定为胰腺轻度缩小,2项或2项以上低者称为胰腺缩小。结果属于轻度缩小者8例,缩小者3例,占总数的45.83%。13例胰腺仍处正常范围内,胰腺前后径大小与病程及慢性并发症之间似无肯定的关系。对老年 NIDDM 的治疗,作者认为若 CT 示有胰头部或2个以上部位前后径缩小,则有用胰岛素治疗的必要。Twenty-four patients with elderly non-insulin-dependent diabetes mellitus(NIDDM)were examined with computed tomography (CT)of pancreas and 107 non-diabetics as control group were compared.The anterio-posterior diameters of pancreatic head.body and tail.and the morphologic structure(contour and density)of pancreatic CT images were investigated comparatively.The results were that the three parts of pancreas in NIDDM were smaller than that of controls.According to the parameter of determination under the normal low range(mean minus 1.96 standard deviation)was decided as diminution,We found that diminution cases in head,body and tail of pancreas is 5,2,and 8 respectively.The contour of pancreatic CT images of elderly NIDDM were incisive and lobulated,and the density of most cases were ununiform.We decided that the anterio-posterior diameter of one part of pancreas under the normal low- est parameter was named as slight diminution.If A-P diameter of two or more than two parts were under the normal lowest parameter it would be named as diminution.In our series,45.83% of total cases of pancreas were reduced in size,in which 8 cases were slight diminition and 3 cases were diminution.The pancreatic size of 13 cases was also within the normal range.The anterio-posterior diame- ter of the three parts of pancreatic CT image had no definite correlation with the duration of disease and its chronic complications.Ac- cording to our observations,we detect from CT examination that if the A-P diameter of the head or more than one parts of pancreas is reduced,those patients with elderly NIDDM might soon require insulin therapy.
