原发纵隔的非何杰金氏淋巴瘤20例临床分析

摘　　要：本文对20例原发纵隔的非何杰金氏淋巴瘤(PMNHL)的常见症状、体征及胸部X线表现作了临床分析。以胸腔积液(75％)、上腔静脉阻塞综合征(65％)、白血病(75％)及中枢神经系统侵犯(30％)的发生率较高。早期多无浅表淋巴结肿大,主要靠胸部X线特点及临床表现作出诊断。我们主张对PMNHL应以包括蒽环类药物在内的联合化疗为主,在病情完全控制之后再辅以局部放疗。其2年存活率为6.8％。The common symptoms, signs and roentgenographic findings of 20 patients with primary mediastinal non-Hodgkin's lymphomas(PMNHL) were studied and discussions were made as to treatment strategy as well as prognosis. It was found that patients with PMNHL had a high incidence of pleural effusion(75％), superior venacaval obstruction (65％), leukemia (75％) and central nervous system dissemination (30％). Because of the aggressive clinicalcourse and poor prognosis, this disease should be treated with combination chemotherapy, When the disease process is under control local irradiation might be supplemented.
