DIAGNOSTIC VALUE OF THE Ag-NOR METHOD IN CELL IMRINTS OF BIOPSY TISSUES OF NASOPHARYNX

摘  要:166例鼻咽活检组织,其中癌99例,炎症64例、结核3例。未治和复发鼻咽癌癌组织印片AgNOR均值分别为14.10±8.22和14.10±5.49粒/核(单位下同),炎症为5.64±1.79和5.20±0.87结核为5.74±0.56。82.83%的癌组织AgNOR均值≥8粒/核,非癌组织只有8.7%。10例2次以上鼻咽部重复活检,最后证明为鼻咽癌6例,另4例最后经颈部活检证明为非鼻咽癌的颈转移癌或恶性淋巴瘤。瘤。他们的炎症组织AgNOR均值均<8,当活检组织为癌时即>8。52.52%癌组织出现1─6个核仁型/核,而非癌组织只有1.49%出现1─2个。各项指标癌与非癌组织对比均有非常显著性差异(P<0.001)。本结果提示,AgNOR检测对鉴别鼻咽部良恶性组织具有重要参考价值,尤其均值≥8者。核仁型可能是鼻咽癌组织印片出现的一个特征。One hundred and sixty-six nasopharyngeal tissue specimens wereexamined,these included 99 nasopharyngeal carcinoma(NPC,92 untreated and 7 recur-rent),52 various benign lesions of head and neck,11 malignant tumors of neck otherthan NPC,1 non-recurrent NPC,and 3 nasopharyngeal tuberculosis. The results showed that:(1) a mean Ag-NOR countper nucleus of untreated and recurrent carcinomatous tissues was 14.1±8.22 and 14.1±5.49(-X±s),respectively,significantly higher than the mean value of 5.64 ± 1.79 and 5.20 ± 0.87 for inflammatory tissues of nasopharynx,also significantly higher than the mean value of 5.74 ± 0.56 for nasopharyngeal tuberculosis(p<0。001). (2) Among 99 carcinomatous tissues,the mean value of 82 cases (82.83%)had overstepped the score of 8 per nuclues(8 per nucleus were nearly the highest value for the inglammatory tissues),among 67 non-carcinomatous tissues only 4(8.7%)overstep- ped the score(p<o.001). (3)10 patients had repeated biopsies for 2-6 times in the nasopharynx,finally 6 haddocumented to be NPC, another 4 were diaghosed to be metastases of neck cancer ormalignant lymphoma. The mean Ag-NOR value of the inflammatory tissue in these pa-tients were < 8.However,those of the carcinomatous tissue were>8.(4)We classified the Ag-NOR into diffused type and nucleolar type. Among the 99carcinomatous tissues,the 1-6 NOR per nucleus had presented in 52.52%(52 /99). Howe-ver,in non-carcinomatous tissues 1-2 NOR per nucleus only in 1.49%(1/67)(p <0.001). The results suggested the frequencies and distributive type of AgNOR in cells ofimprint of biopsied tissues in the nasopharynx may have important value for the diffe-rential diagnosis between benign and malignant tissues. especially,when the mean valueof AgNOR was more than 8 per nucleus,nasopharungeal carcinoma should be conside-red.The nucleolar type may be one of the feature of malignant tumors on the imprintof tissues.
