眼睑恶性肿瘤切除后自体搭桥式皮瓣再造术

摘　　要：目的　探讨眼睑恶性肿瘤切除后,经睑缘搭桥行自体再造术的可行性和可靠性。方法　6例眼睑恶性肿瘤患者行肿瘤切除术后,根据术后眼睑缺损面积大小,其中3例上眼睑恶性肿瘤患者先行下睑缘搭桥术,利用自体全层下睑再造上睑,3个月后再行眼裂再造术;3例下眼睑恶性肿瘤患者行上睑灰线切开,利用睑板结膜瓣和下睑切口的内外眦部皮肤游离瓣共同重建下眼睑。结果　6例术后上下眼睑结构正常,血供良好,闭合完全,活动自如,外观基本恢复原样或睑裂较原来大,皮肤皱纹消除。除肿瘤切除部位睫毛缺损外,术后无并发症发生。结论　搭桥术自体再造术可修补眼睑大面积缺损,弥补眼睑恶性肿瘤切除术给患者生理和心理造成的创伤。Objective To investigate the probability and reliability of eyelid rebuild operation via palpebral margin after resecting the tumour in patients with eyelid's malignant tumour.Methods Base on the blepharocoloboma sizes of eyelid,after the tumour resection operated in 6 malignant tumour patients,3 patients of upper-eyelid malignant tumour were operated with surgery from the nether eyelid,using the whole layers of nether self-eyelid to rebuild the upper eyelid,then palpebral fissure's rebuild operation were carried after three months.Upper-eyelid in other 3 patients of nether-eyelid malignant tumour were carved,rebuilding the nether-eyelid by conjunctival flap and free flap of cutis from inside and outside canthus.Results Upper and nether eyelid's structure were natural in 6 patients,blooding supply very well,close entirely.The appearance of eyes resumed to normal basicly.There were no any syndrome except that the resecting area had no eyelash.Conclusion This operation can restore the big blepharocoloboma sizes and remedy the soul wound from the tumour resection in patients.
