摘  要:目的 观察渗出型老年性黄斑变性 (AMD)的吲哚青绿血管造影 (ICGA )特征。方法 对 2 7例 (32只眼 )渗出型 AMD行 ICGA造影或 (和 )眼底荧光血管造影 (FFA)检查 ,并对二者的造影表现特点和差异进行比较。结果  FFA检查 2 9只眼 ,诊断典型性 CNV11只眼 (37.93% ) ,表现为黄斑部早期有网状强荧光性 CNV,后期荧光素不断渗漏 ,并积存在 RPE脱离范围之内。晚期形成明显区域性强荧光。诊断隐匿性 CNV 18只眼 (6 2 .0 7% ) ,表现为早期无 CNV特征 ,晚期有不明原因的边界不清强荧光 5只眼 (17.2 4 % ) ,而表现早期有边界模糊强荧光呈进行性渗漏 ,晚期强荧光区明显扩大 13只眼 (44 .82 % )。 ICGA检查 32只眼 ,诊断典型性 CNV32只眼 (10 0 % ) ,表现为早期有强荧光性 CNV性 11只眼 (34.37% ) ,中期出现强荧光性 CNV15只眼 (46 .88% ) ,晚期又出现新的强荧光性CNV 6只眼 (18.75 % )。表明 FFA和 ICGA在渗出型 AMD的 CNV的诊断比率上 ,二者有显著差异。ICGA比 FFA能更精确地显示黄斑部的 CNV ,而对于黄斑部视网膜下出血、浆液性色素上皮脱离等视网膜病变则不如 FFA显示明确。结论  ICGA能发现 FFA不能发现的 CNV,在显示 CNV的数量、位置、范围等方面比 FFA敏感和准确 ,是诊断渗出型 AMD的 CNV和指导其激光光凝的?Objective To investigate the characteristics of the fundus indocyanine green angiography(ICGA) for exudative aged macular degeneration (AMD). Methods Thirty-two eyes of 27 patients with AMD were selected for ICGA and fundus fluorescein angiographyh (FFA).The characteristics of the two examinations were compared for the difference. Results Twenty-nine eyes had FFA, 11 of them had been diagnosed as CNV(37.93%), displayed CNV with reticular strong fluorescence of macular at early stage, continued to exudates flurescence at late stage, accumulated within the range of RPE formed local significant strong fluorescence at late stage. The others 18 eyes were diagnosed as latent CNV (62.07%), five (17.24%) of them did not show the characteristics of CNV at early stage and had strong fluorescence with unclear border and unidentified cause. The others 13 eyes (44.82%) exhibited blur border strong fluorescence and continued to exudates at early stage and the range of the strong fluorescence were significantly larger. Thirty-two eyes received ICGA and all of them were diagnosed as typical CNV, 11 (34.37%) of them manifested CNV with strong fluorescence at early stage, 15 (46.88%) showed CNV with thick fluorescence at middle stage and 6 (18.75%) of them showed CNV with new strong fluorescence. The diagnosed ratios of CNV of exudative AMD were significantly different between FFA and ICGA. ICGA showed macular CNV more accurately then FFA. Showing the hemorrhage under retina and the detachment of serous pigment epithelial layer, ICGA was not as well as FFA.Conclusion ICGA can show the CNV which FFA can not. ICGA is more sensitive and accurate than FFA to show the numerous, location and range of CNV. So ICGA is an important method to diagnose exudative AMD and to direct laser coagulating.
