高度近视全阈值静态视野形态改变及相关因素分析

摘  要:目的:研究高度近视眼全阈值静态视野形态的改变。方法:应用Dicon TKS-5000型自动视野分析仪检查高度近视65例(130眼)的全阈值静态视野,并观察其后极部眼底改变。结果:视野异常115眼(88.5%),其中生理盲点扩大86眼(66.2%),出现黄斑中心相对暗点75眼(57.7%),弥漫性损害39眼(30.0%)。发现黄斑中心相对暗点75眼中仅有51眼在眼底镜下发现黄斑部病变(χ2=22.042,P<0.01)。结论:高度近视眼的视野改变与患者的屈光度及后极部眼底病变有关。视野改变的形态,程度与眼底后极部改变的形态不完全对应。高度近视患者视野改变早于眼底镜下眼底改变,对于早期诊断及治疗具有指导意义。AIM: To study the changes of automated static visual field in high myopia.· METHODS: The static visual fields in 65 cases (130 eyes) with high myopia were tested by Dicon TKS-5000 Field Analyzer and their posterior polar lesions were observed.· RESULTS: Among the 130 eyes, 115 eyes (88.5%) showed field abnormalities. Enlargement of the blind spot occurred in 86 eyes (66.2%), central relative scotomata in 75 eyes (57.7%), and diffuse field defects in 39 eyes (30.0%). Among the 75 eyes of central relative scotomata,only 51 eyes showed macular lesions under the ophthalmoscope (χ2=22.042, P <0.005).· CONCLUSION: The visual field defects in the high myopic eyes relate to the degree of high myopia,the age of the patients and the severity of the posterior polar lesion. The appearance of visual field abnormalities in high myopic eyes detected by a quantified automated perimeter is prior to the appearance of the posterior polar lesions seen under the ophthalmoscope. The automated visual field examination is helpful for early diagnosis and treatment of high myopia.·
