超声乳化白内障吸除人工晶状体植入治疗恶性青光眼

摘　　要：目的:观察超声乳化白内障吸除后房型人工晶状体植入治疗恶性青光眼的临床疗效。方法:对18例(18眼)青光眼滤过手术后发生的恶性青光眼,超声乳化白内障吸除、后房型人工晶状体植入、小梁切除。结果:术后前房不同程度加深,眼压控制正常,术前眼压:5.47±1.53kPa,术后眼压:2.31±0.32kPa。视力不同程度提高,<0.05者1眼(5.5%),0.1～0.3者11眼(61.1%),>0.4者7眼(38.9%)。并发症:瞳孔区渗出膜5眼(27.8%),虹膜后粘连6眼(33.3%)、前粘连2眼(11.1%),角膜内皮水肿6眼(33.3%)。结论:超声乳化白内障吸除后房型人工晶状体植入是治疗恶性青光眼的有效方法之一。AIM: To evaluate the clinical effects of phacoemulsification cataract extraction combined with posterior chamber intraocular lens implantation in the treatment of malignant glaucoma.· METHODS: For the 18 cases (18 eyes) with malignant glaucoma occured after glaucoma filtering operation, phacoemulsification for cataract extraction, posterior chamber intraocular lens implantation and trabeculectomy were performed.· RESULTS: The anterior chamber depth increased to different degrees after operation. The pre-operative intraocular pressure (IOP) was 5.4±71.53kPa and the postoperative IOP was 2.31±0.32kPa. The visual acuities were improved as: lower than 0.05 in 1 eye (5.5%), 0.1-0.3 in 10 eyes (55.6%) and higher than 0.4 in 7 eyes (38.9%). Complications included exudative iris in papillary area in 5 eyes (27.8%), posterior synechia in 6 eyes (33.3%), anterior synechia in 2 eyes (11.1%) and edematous corneal endothelium in 6 eyes (33.3%).· CONCLUSION: The combination of phacoemulsification cataract extraction and posterior chamber intraocular lens implantation is an effective method to treat malignant glaucoma.·
