颞浅动脉插管化疗治疗72例晚期口腔颌面部恶性肿瘤的临床观察

摘　　要：目的:观察口腔颌面部晚期恶性肿瘤颞浅动脉插管化疗的近期疗效、影响因素及毒副反应.方法:对72例口腔颌面部晚期恶性肿瘤采用TDP方案进行插管化疗,THP 20 mg, 静脉推注,d1;DDP 30 mg,静脉推注,d2～d4;PYM 8 mg,静脉推注,d5～d9.21 d为1个周期,满2个周期以上进行疗效评价.结果:总有效率(CR+PR)为90.28%(65/72),其中 CR为16.67%(12/72),PR为73.61 %(53/72).Ⅲ期有效率93.75%(30/32),Ⅳ期有效率87.50%(35/40),两组比较差异无统计学意义,P=0.62. 初治的有效率100.00%(38/38),复治的有效率为79.41%(27/34),两组比较差异有统计学意义,P=0.01.化疗2个周期有效率为86.36%(38/44),化疗＞2个周期有效率为96.43%(27/28),两组比较差异无统计学意义,P＞0.32.结论: 颞浅动脉插管化疗对晚期口腔颌面部恶性肿瘤疗效好,安全且不良反应轻.初治与复治是影响疗效的主要因素.OBJECTIVE:To evaluate the effect, relevant factors and adverse reactions of temporal artery intubation chemotherapy in the treatment of advanced oral maxillofacial carcinoma.METHODS: Seventy-two cases of advanced oral maxillofacial carcinoma were treated with combination chemotherapy (THP DDP PYM) in this study.THP 20 mg,arterial injection,d1,DDP 30 mg, arterial injection,d 2-d 4 and PYM8 mg, arterial injection,d 5-d 9,21 days were 1 cycle,and at least 2 cycles were given.RESULTS: The total effective rate(CR+PR) was 90.28%(65/72), CR:16.67%(12/72),PR:73.61 %( 53/72).The effective rates of stage Ⅲ and stage Ⅳ were 93.75% ( 30/32) and 87.50%(35/40) respectively, and there was no statistical difference between the two groups, P>0.05.The effective rates of preliminary and recurrent patients were 100.00%(38/38) and 79.41%(27/34), respectively, and there was statistical difference between the two groups, P<0.05. The effective rates of patients treated 2 cycles and >2 cycles were 86.36%(38/44) and 96.43%(27/28), respectively, there was no statistical difference between the two groups, P>0.05. CONCLUSIONS: Temporal artery intubation chemotherapy on advanced oral maxillofacial carcinoma has better effect, safety and slightly adverse reactions. Preliminary or recurrence are important factors that affect the effective rate.[
