摘　　要：【目的】通过 6 5例岩骨尖斜坡区脑膜瘤临床外科治疗的疗效分析 ,探讨显微手术切除岩斜区脑膜瘤的手术方法及术后疗效 ,达到提高显微手术的全切率 ,总结临床治疗经验。【方法】熟练掌握肿瘤与岩骨尖斜坡区周围解剖关系 ,总结 6 5例岩骨尖斜坡区脑膜瘤的临床表现 ,手术入路 ,手术结果和术后处理。【结果】肿瘤全切除 5 0例 (77% ) ,死亡 4例 (6 .1% )。经岩骨乙状窦前幕上下联合入路 30例 ,全切除 2 0例 (6 6 .7% ) ,死亡 3例 (10 % )。【结论】选择好手术入路和运用好显微外科技术可以提高岩骨尖斜坡区脑膜瘤的切除率 。Objectives]To discuss the operative approach and postoperative outcome of petroclival meningioma (PM) treated by microscopic surgery for the aim of improving microneurosurgical techniques and summarize the clinical experiences of treatment.As the anatomical relation of tumor to the region around petroclivus was reviewed, the clinical manifestations, operative appraches, outcome of operation and postoperative management in 65 patients with PM were summarized.Total resection of PM was achieved in 50 cases (77%). Operative death occurred in 4 cases (6.1%). Thirty cases with PM were operated through combined presigmoid retrolabyrinthine approach. Their total removal was achieved in 20 cases (66.7%), with operative death in 3 cases (10%).[Conclusion]Total resection rate can be raised by proper operative approaches and skillful manipulation of microneurosurgical technique. Strengthening of postoperative clinical treatment can reduce mortality of operation.
