摘  要:【目的】探讨急性氟乙酰胺中毒患者血清酶及心电图变化的临床意义。【方法】4 4例不同程度中毒患者入院 6h内抽取静脉血检测磷酸肌酸激酶 (CPK)、磷酸肌酸激酶同工酶 (CPK MB)、天冬氨酸氨基转移酶 (AST)、乳酸脱氢酶 (LDH)的活性及心电图检查 ,并与同期 4 0名健康人血清酶及心电图检测作对照。【结果】中毒患者血清酶较对照组均有显著差异 (P <0 .0 5 ) ,三组间酶值比较 ,中度组明显高于轻度组 ,重度组明显高于中度组 ,差异有显著性 (P <0 .0 5 ) ;患者心电图异常与中毒程度相关。【结论】氟乙酰胺中毒血清酶活性变化可用于判断病情和估计预后 ,心电图异常更能提示心肌损害程度 ,对治疗具有指导意义。Objective To explore the clinical significance of changes in serum enzyme activity and electrocardiogram in patients with acute fluoroacetamide poisoning (AFP).Venous blood samples were collected from 44 patients with different degree of AFP (mild, moderate and severe grades) within 6 hours after admission, the activities of their serum creatine phosphokinase(CPK), CPK-MB, aspartate aminotransferase(AST) and lactate dehydrogenase (LDH) were estimated and electrocardiograms were examined. Their results of examination were compared with those of 40 healthy subjects served as controls.There were significant differences in serum enzyme activities between patients and control group (P<0.05). The serum enzyme activities in moderate poisoning group were significantly higher than those of mild poisoning group ; whereas the serum enzyme activities in severe poisoning group were obviously higher than those in moderate poisoning group (both P<0.05). Abnormality of electrocardiograms in patients was correlated to the degree of poisoning.[Conclusion]The changes of serum-enzymes activity may be used to judge the severity of disease and assess the prognosis in AFP. Abnormal electrocardiogram can suggest the degree of myocardial damage and thus guide the treatment of patients.
