摘 要:目的:探讨胰腺癌中p21W A F1、cyclinD1和CD K4的表达意义及其相互关系。方法:应用免疫组织化学二步法检测p21W A F1、cyclinD1和CD K4在48例胰腺癌和14例慢性胰腺炎中的表达。结果:胰腺癌组织中p21W A F1、cyclinD1、CD K4的阳性表达率分别是52.08%(25/48)、70.85%(34/48)、62.50%(30/48),同慢性胰腺炎组比较p21W A F1的表达明显降低(P<0.01),而cyclinD1和CD K4的表达明显升高(P<0.01)。高分化、无淋巴结转移、临床分期Ⅰ、Ⅱ期的胰腺癌组织中p21W A F1的阳性表达率明显高于低分化、有淋巴结转移、临床分期Ⅲ、Ⅳ期的胰腺癌(P<0.05);而cyclinD1和CD K4则相反(P<0.05)。p21W AF1的表达和cyclinD1、CDK4的表达呈明显的负相关(相关系数分别为r=-0.340,P<0.05和r=-0.571,P<0.01);cyclinD1和CDK4的表达则呈明显的正相关(r=0.450,P<0.01)。结论:p21W A F1的缺失表达和cyclinD1、CD K4的过表达在胰腺癌的发生、发展过程中起重要的协同作用。三种蛋白质主要以p21W AF1/cyclinD1/CDK4通路的方式作用于细胞的转化,参与肿瘤的形成。Objective: To investigate the expression of p21WAF1 cyclinD1 CDK4 and their relationship in pancreatic cancer. Methods: Immunohistochemical detector with new second generational tow steps method was used to detect the expression of p21WAF1 cyclinD1 CDK4 in 48 cases of pancreatic cancer and 14 cases of chronic pancreatitis. Results: The expression of p21WAF1 cyclinD1 CDK4 proteins in pancreatic cancer were 52.08% (25/48) 70.85% (34/48) and 62.50% (30/48), respectively. The p21WAF1 proteins was reduced in pancreatic cancer (P<0.01), while the cyclinD1 and CDK4 proteins were increased (P<0.01), as compared to that in chronic pancreatitis. The positive rates of p21WAF1 in the pancreatic cancer, with well-differentiation, non-metastatasis and clinical stageⅠ/Ⅱ, was significantly higher than that in the cases with poor differentiation, metastasis and clinical stage Ⅲ/Ⅳ(P<0.01), and that of cyclinD1 and CDK4 were opposite(P<0.05). The negative correlation was found between the expression of p21WAF1 and cyclinD1 CDK4(r=-0.340, P<0.05; r=-0.571, P<0.01).The positive correlation was also found between cyclinD1 and CDK4(r=0.450, P<0.01). Conclusion: The loss of p21WAF1 expression and overexpressions of cyclinD1 and CDK4 may contribute to the pathogenesis of pancreatic cancer. These proteins play a critical role in cell cycle arrest through p21WAF1/cyclinD1/CDK4 pathway.
