PPARγ2基因Pro12Ala多态性与老年人高血压合并脑梗死关系的探讨

摘　　要：目的探讨过氧化物酶体增殖物激活受体γ2(PPARγ2)基因Pro12Ala多态性与老年人高血压合并脑梗死及血脂的关系。方法随机选取湖北地区汉族老年人165例,其中正常对照组95例,高血压合并脑梗死组70例。正常非老年人125例。应用聚合酶链反应限制性片段长度多态性技术,进行基因型检测,并用直接测序法加以证实。结果正常老年人PPARγ2基因Pro12Ala多态性的基因型及等位基因频率分布,与非老年人群相似(P>0.05)。PPARγ2基因Pro12Ala多态性与老年人高血压合并脑梗死的发病无关(P>0.05)。在老年脑梗死患者中,PA/AA基因型者的血清低密度脂蛋白胆固醇(LDLC)水平〔(3.47±0.70)mmol/L〕,较PP型者〔(2.54±0.39)mmol/L〕显著升高(P<0.05)。该变异与血压、血糖、体重指数无关(均为P>0.05)。结论PPARγ2基因Pro12Ala多态性可能不是老年人高血压合并脑梗死发病的遗传学因素。但该变异参与脂质异常的调节,脑梗死患者中,PA/AA型者的血清LDLC水平,较PP型者显著升高。Objective To explore the relationship between peroxisome proliferator-activated receptor (PPAR)γ2 gene Pro12Ala polymorphism and hypertension complicated with cerebral infarction (CR) and blood lipid. Methods 165 Han nationality elderly from Hubei were randomly selected, including 95 cases in normal control group, 70 cases in hypertension complicated with CR group. 125 cases of normal non-elderly were also contained in this study. PPARγ2-Pro12Ala gene variant was detected using polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP) and validated by direct sequencing. Results Polymorphism and allele frequency distribution of PPARγ2 gene Pro12Ala in elderly people were similar to those in non-elderly people (P>0.05). The Pro12Ala polymorphism was not significantly related to the occurrence of hypertension complicated with cerebral infarction (P>0.05). Among patients with cerebral infarction, the level of LDL-cholesterol 〔(3.47±0.70)mmol/L〕 in PA/AA genotype ones was significantly higher than that in PP genotype ones 〔(2.54±0.39)mmol/L,P<0.05〕, which had no relation to blood pressure, body weight index (BMI) and blood sugar (P>0.05). Conclusions Pro12Ala polymorphism of PPARγ2 gene might not be genetic factor of the occurrence of hypertension complicated with cerebral infarction in the elderly. But it could participate in the regulation on abnormal level of blood lipid, serum LDL-C level in PA/AA genotype patients with cerebral infarction could significantly increase than that in PP genotype ones.
