外伤性晶状体脱位继发青光眼的手术治疗——晶状体玻璃体切除联合小梁切除术的效果观察

摘　　要：目的　观察外伤性晶状体脱位继发青光眼行晶状体玻璃体切除联合手术的临床效果。方法　 3 1例 (3 1眼 )外伤性晶状体脱位继发青光眼中 ,2 6眼行睫状体平坦部三通道闭合式晶状体玻璃体切除联合小梁切除术 ,除 1例外均于术中联合后房人工晶状体植入缝合固定术。另 5眼行角膜缘切口晶状体囊内摘出联合前部玻璃体切除及小梁切除术。结果　术后视力 0 .1～ 0 .3者 5眼 ;0 .4～ 0 .6者 11眼 ;0 .7及以上者 15眼。术后 3 0眼眼压恢复到正常范围 ,1眼眼压控制不良 ,再行抗青光眼硅管植入术后 ,眼压恢复正常。结论　外伤性晶状体脱位继发青光眼作晶状体玻璃体切除联合手术是安全且有效的手术。Objective To investigate the clinical efficacy of combined surgical approaches in management of traumatic dislocated lens and secondary glaucoma. Methods 26 eyes were treated with lensectomy, vitrectomy and trabeculectomy.25 of the 26 eyes combined with posterior chamber intraocular lens implantation. The other 5 eyes were treated with extraction of lens through a limbal incision,anterior vitrectomy and trabecuectomy. Results 5 eyes got 0.1-0.3 visual acuity, 11 eyes got 0.4-0.6 visual acuity, 15 eyes got 0.7 or better visual acuity after surgery. Intraocular pressure (IOP) of 30 eyes were normal. IOP of 1 eyes wasn't controlled, and it was normal after silicon tube implantation. Conclusions For traumatic dislocation of lens and secondary glaucoma, the combined operation of different surgical approaches is effective and safe.
