正常人脑组织对称部位平扫CT值差值的测量在超急性期脑梗死诊断中的应用价值

摘　　要：　　目的　通过对正常人脑组织对称部位CT值的测量差值研究,寻求正常参考值,以探讨其在超急性期脑梗死CT诊断中的应用价值。方法　用常规CT扫描对 33例神经系统正常志愿者的 7处脑组织对称部位分别进行CT值测量,计算差值(ΔHU)共 231个,对所测得数据进行统计学分析得出正常参考值,将其与经临床追踪证实的超急性期脑梗死组 30例 (33个可疑病灶)的对称部位CT值差值进行比较。结果　正常人脑组织对称部位CT值测量差值的正常参考值平均为M=0. 39(ΔHU),四分位数间距Q=0. 48(ΔHU),最大值为 1. 79 (ΔHU),最小值为 0. 00 (ΔHU);病变组CT值测量差值平均为M=3. 62(ΔHU),Q=2. 44(ΔHU),最大值 8. 34(ΔHU),最小值 1. 2(ΔHU)。正常组 7处脑组织对称部位差值均值与病变组比较,有显著性差异 (Ρ<0. 01)。结论　正常组的CT值测量差值 96. 97%为 1. 2 (ΔHU)以下,是异常组的最小值,正常组的最大值 1. 8 (ΔHU)。应用正常中老年人脑组织对称部位平扫CT值测量差值对评价超急性期脑梗死有重要参考价值。Objective To obtain the normal reference value through researching ΔHU of CT scanning symetric cerebral structrue among healthy group and to study its value in CT diagnosing super acute cerebral infarction.Methods 33 volunteers with normal neuro system were undertaken routine plain CT scanning to measure the symetric cerebral structrue on 7 pairs of tissues of each person and caculated 231 ΔHUs. All the data were analyzed statistically to get a normal reference value, which was compared with the ΔHU of 30 super acute cerebral infarction cases (33 suspected symptoms)confirmed by follow-up.Results The meanΔHU of CT scanning symetric cerebral structrue among healthy group was 0.39 with Q=0.48(ΔHU) a maximum of 1.79 (ΔHU) and minimum of 0.00 (ΔHU). Among the pathological change group, the mean ΔHU of CT value was 3.62 with Q=2.44 (ΔHU), a maximum of 8.34(ΔHU) and minimum of 1.2 (ΔHU). There was significant difference between the ΔHUs of 7 pairs of cerebral tissues of each person in normal group and those of the pathological change group(P<0.01).Conclusion The ΔHU of CT value is 1.2 in 96.97% of the normal group and it is the minimum in the abnormal group.The maximum is 1.8(ΔHU) in normal group.This ΔHU of CT value is of reference significance in evaluating the super acute cerebral iufarction.
