摘 要: 目的 探讨小肝癌的生物学特性,为SHCC的治疗和预后判断提供新的依据。方法 手术病理证实小肝癌 30例,共 33个癌结节,术前均进行螺旋CT多期扫描,其中男 26例,女 4例。对SHCC血供进行SCT分型。术后肿瘤行大体病理及其HE染色、免疫组化染色分析。结果 动脉供血型 19个病灶,门脉供血型 0个病灶,双重供血型 7个病灶,少血供型 7个病灶。各种供血型肿瘤间p53表达阳性率无显著性差异(Ρ>0. 05);动脉供血型、双重供血型与少血供型之间p21表达阳性率差异有显著性( (Ρ<0. 05或<0. 01);动脉供血型或动脉供血型和双重供血型合并组与少血供型之间VEGF表达阳性率差异有显著性(Ρ<0. 05 )。动脉供血型和双重供血型的小肝癌,其MVD记数均高于少血供型 (Ρ<0. 01 )。结论 少血供型SHCC组织中VEGF、p21表达阳性率高于多血供型(动脉供血型和双重供血型),而MVD则相反。血供类型与p53表达无关。Objective To study the biological behavior of small hepatocellular carcinoma (SHCC) for new approaches to treatment and predicting prognosis of SHCC.Methods Multiphase spiral CT scans were performed in 30 patients with 33 lesions of SHCC (≤3.0 cm in diameter) confirmed histopathologically. Of them,26 were male and 4 were female. Blood supply of SHCC was divided into four types on multiphase SCT. Immunohistochemical and HE staining of surgical samples in all patients were performed.Results The masses were supplied by arterial blood in 19 by double blood in 7,lack of blood supplies in 7 and non-supply by portal blood was seen.Positive rate of p53 was not correlated with tumor blood supplies (Ρ>0.05).Singnificant difference of p21 positive rates was showed between tumor lack of blood supplies and arterial blood (Ρ<0.05), double blood (Ρ<0.05), arterial combining with double blood supplying (Ρ<0.01) respectively. There was statistic difference of VEGF positive rates between arterial blood, or arterial combining with double blood supplying and poorly blood supplying SHCC respectively (Ρ<0.05). In SHCC with arterial and double blood supply, MVD expression level was higher than that with poor blood supplies(Ρ<0.01).Conclusion Positive rate of VEGF and p21 is higher in poorly blood supplying SHCC than that in rich blood supplies one (arterial and double blood supplying), while the MVD statue is an opposite result. Tumor blood supply is not correlated with positive rate of p53.
