摘 要:目的:观察大鼠脑出血后血肿周围神经元线粒体内细胞色素C释放规律,血肿变化及中药复方血府逐瘀汤的干预作用。方法:实验于2003-10在湘雅医院中西医结合研究所进行。用Ⅶ胶原酶脑内立体定位注射诱导大鼠脑出血模型。免疫组织化学法检测神经元线粒体内细胞色素C的释放情况,同时测量不同时间点大鼠血肿的最大直径。结果:大鼠脑出血后血肿周围神经元于6h即有细胞色素C释放,出现胞浆染色的阳性细胞;1d后阳性细胞表达最多;3d后阳性细胞数减少;5d后阳性细胞继续减少。治疗组与模型组比较在1,3,5d时阳性细胞减少明显(P<0.01)。造模后血肿于1d达到最大,3d有所变小,5d变小明显;治疗组在6h与模型组比较血肿缩小明显(P<0.05),1,3,5d缩小更明显(P<0.01)。结论:脑出血后血肿周围神经元细胞色素C于6h即开始从线粒体释放,1d达高峰后下降;血府逐瘀汤能阻制细胞色素C的释放,同时促进血肿的吸收。AIM:To observe the effect of xuefu zhuyu decoction (XFZYD) on the release of cytochrome C(Cyt- C) from neuronal mitochondria around the hematoma after intracerebral hemorrhage(ICH) and the changes of hematoma in rat and study the releasing regularity of Cyt- C. METHODS:The experiment was performed in Institute of Combined Traditional Chinese and Western Medicine of Xiangya Hospital in October 2003.Ⅶ type collagenase was injected into rats with stereotaxic technique to build the ICH model.The immunohistochemical methods was used to exam the release of Cyt- C from neuronl mitochondria after ICH in rat.And ruler was used to measure the biggest diameter of rat hematoma at different time. RESULTS:Cyt- C began to release in the neuron around the hematoma after ICH in rat at 6 hours after operation,there were some positive cells that were stained at the cytoplasm around the hematoma; after 1 day,the number of positive cells reached to the peak.But after 3 days,the positive cells decreased;after 5 days,the positive cells continued to decrease.The number of positive cells of the XFZYD group was obviously smaller than that of the model group(P< 0.01).The hematoma swelled to the biggest at 1 day,deflated lightly at 3 days,and became much smaller at 5 day.The hematoma of treatment group deflated obviously smaller than that of model group at 6 hours(P< 0.05),it deflated more obviously at 1 day,3 days and 5 days(P< 0.01). CONCLUSION:The release of cytochrome C begins at 6 hours from neuronal mitochondria around the hematoma after ICH;it reaches to the peak at 1 day and then decreases.XFZYD can markedly depress the release of Cyt- C and accelerate the absorption of hematoma.
