摘 要:目的 :通过电镜观察胆固醇结石内的细菌形态学变化 ,探索细菌在成石中的作用。方法 :选择接受手术治疗的病人共计 4 6例 ,男 2 4例 ,女 2 2例 ;平均年龄 5 4 .6岁 (35~ 72岁 ) ;均确诊为胆石症。其中胆囊结石 2 0例 ,肝内外胆管结石 12例 ,胆总管结石并胆囊结石 14例。利用扫描电镜、透射电镜对胆固醇结石进行观察。结果 :胆固醇结石表层结构内均能够发现细菌 ,胆固醇结晶中少有细菌 ,结石核心可发现细菌。结论 :对胆固醇结石进行扫描电镜、透射电镜观察 ,发现胆固醇结石中有细菌存在 ,细菌存在于结石的核心和外周 ,说明细菌在胆固醇结石的成因和发病机制中可能充当始动因子作用。Objective:To observe the morphological change of bacteria in the cholesterol stone under electron microscope and to explore the action of bacteria in the formation of stone.Methods:Forty-six patients were undergone operation;of which,24 cases were male,22 cases female,average age 54.6 (35~72). All patients were diagnosed as cholelithiasis. 20 of them were cholecystoliathiasis,12 of them extrahepatic and intrahepatic bile duct stone. Fourteen of them were CBD stone combined with gallstone. The cholesterol stones were observed with scanning electron microscope and transmission electron microscope.Results:Bacteria were found in the superficial layer of the cholesterol stone,seldom found in the cholesterol crystal and could be seen in the core of cholesterol stone yet.Conclusion:Bacteria are found in the cholesterol stone and in the core and the periphery of the cholesterol stone through the observation with scanning electron and transmission electron microscope. It suggests that bacteria act as an initial factor in the formation of the gallstone and pathogenesis.
