乳腺黏液腺癌的细针吸取细胞学诊断价值及标准

摘　　要：目的　探讨细针吸取细胞学(FNAC)在诊断乳腺黏液腺癌中的价值及标准。方法　对14例乳腺黏液腺癌FNAC涂 片的基本形态学要素进行回顾性比较研究,并对细胞学诊断为Ⅰ～Ⅲ级的病例进行原因分析。结果　乳腺黏液腺癌核级主为 1～2级,属轻～中度异型性,诊断敏感性为71.4%,假阴性率为28.6%。14例均出现多少不等的黏液,12例出现三维结构为 主以及弥散为主的细胞排列方式,11例出现中度异型的游离肿瘤细胞。乳腺纯黏液腺癌组4例出现砂粒体。结论　只要掌 握乳腺黏液腺癌FNAC涂片基本形态特征,FNAC诊断乳腺黏液腺癌是可行的。Purpose To investigate the diagnostic value and standards of fine needle aspiration cytology(FNAC) for diagnosis of mucinous breast carcinoma. Methods The basic morphological characteristics of the FNAC smears of 14 cases mucinous breast carcinoma were compared. The reasons of the diagnosed cases( grade Ⅰto Ⅲ)were analyed. Results The cell nuclei grades of mucinous breast carcinoma were mainly from grade one to grade two,belonging to light or medium dysplasia. The diagnosed sensitivity was 71.4%. False negative rate was 28.6%. There were more or less mucus appeared in all the 14 cases. Cells arranged in two main types,tri-dimension structure or dispersional pattern in 12 cases. Dissociated tumour cells of medium dysplasiac appeared in 11 cases. There were 4 cases of psammoma bodies among the group of pure mucinous breast carcinoma. Conclusions Mucinous breast carcinoma can be diagnosed in FNAC smears by the basic morphological characteristics.
