喉癌手术切缘Bcl-2、Bax及Bak蛋白的表达

摘　　要：①目的探讨喉癌手术安全切缘的范围.②方法采用免疫组化SP法,检测60例喉鳞状细胞癌病人的癌组织、距离癌组织边缘2、5、10 mm组织和20例声带息肉黏膜对照组织切片中Bcl-2、Bax和Bak蛋白的表达.③结果 3种癌基因蛋白产物阳性表达按正常黏膜,距癌灶10、5、2 mm,癌组织顺序依次增高,越靠近癌组织蛋白表达越强,差异有极显著性(χ2=11.87～34.29,P<0.01).④结论喉癌手术可以距肿瘤5 mm作为安全切缘的参考标准.Objective To study the range of safe margins of excision in operation for cancer of larynx. Methods The specimens of 60 cases of squamous cell carcinoma(LSCC) were studied, which included the cancer tissue, tissue which was 2 mm,5 mm,and 10 mm away from cancer. Twenty mucosa specimens taken from vocal cord polyp served as the control. The Bcl-2, Bax and Bak were detected by immunohistochemical method of SP. Results The positive expression of the three proteins increased in sequence of normal mucosa, those at 10 mm, 5 mm, 2 mm adjacent to carcinoma and cancer tissue. The nearer to the carcinoma mucosa, the more the proteins were expressed (χ2=11.87-34.29,P<0.01). Conclusion The excision margin at 5 mm away from the tumor can be regarded as a safe range for surgery of cancer of larynx.
