摘 要:目的:探讨胃癌恶病质病人血清瘦素水平的变化及其与机体组成之间的关系。 方法:进展期胃癌病人 53例,按与发病前比较体重下降的百分比,将病人分为恶病质组(Ⅰ组)和非恶病质组 (Ⅱ组 ),测定血清瘦素、胰岛素水平和机体组成。 结果:恶病质组病人的瘦素水平明显低于非恶病质组 (P=0. 024),血清瘦素水平与体质指数(BMI)、机体脂肪群、血清胰岛素水平均呈正相关,其中与机体脂肪群的相关性最高 (r=0. 626,P<0. 0001 ),而与机体瘦组织群、肌肉群、蛋白质群则呈负相关。 结论:血清瘦素水平可以反应胃癌恶病质病人的营养状况,作为营养不良的诊断指标之一。Objectives: To investigate the change of serum leptin and its relation with body composition data in gastric cancer cachexia patients. Methods: 53 gastric cancer patients were dividedinto two groups(cachexia group and non-cachexia group) on the basis of their weight change. Their serum leptin, insulin and body composition were detected.The relationship between leptin and other values were analyzed. Results: The leptin level of cachexia group patients was significantly lower than non-cachexia group (P=0.024). The insulin level, BMI, lean body mass, muscle mass, protein mass and fat mass were all significantly correlatedto leptin level (for fat mass, r=0.626, P<0.0001). Conclusions: The leptin level in cancer cachexia patients is not increased abnormally, but canact as a laboratory value to detect malnutrition.
