API2-MALT1融合基因表达与结外弥漫大B细胞淋巴瘤临床病理特征与预后的关系

摘　　要：目的检测结外弥漫大B细胞淋巴瘤API2MALT1融合基因mRNA表达,了解其表达与结外弥漫大B细胞淋巴瘤临床病理特征与预后的关系。方法收集胃肠和甲状腺结外弥漫大B细胞淋巴瘤共32例,通过RT PCR检测所有病例的API2MALTlmRNA,收集临床病理资料和随访。根据融合基因表达情况将病例分为两组,即API2MALT1mRNA表达与未表达组,比较两组的临床病理特征和预后。结果32例中11例检出融合基因表达(34.4%)。与API2MALT1mRNA未表达病例相比较,API2MALT1mRNA表达病例临床病理分期较早,细胞增殖指数、淋巴结累及远处转移率和复发率都较低,两组间差异有显著意义。生存分析:API2MALT1mRNA表达病例平均存活时间长、5年生存率高,两组的存活情况差异有显著意义。结论API2MALT1融合基因表达对结外DLBCL的临床病理特征和预后有明显影响,来源于MALT淋巴瘤恶性转化的DLBCL有具有更加惰性的临床过程和较好的存活情况。MALT淋巴瘤和由它高恶性转化而来的结外DLBCL应被视为同一个临床病理学实体,这样更能体现MALT淋巴瘤的演进过程。Objective To detect API2-MALT1 mRNA of extranodal diffuse large B-cell lymphoma(DLBCL) and to investigate the correlation between the expression of API2-MALT1 mRNA and clinicopathological features and prognosis in extanodal DLBCL.Methods 32cases of extranodal DLBCL were retrieved. API2-MALT1 mRNA was detected in all case by RT-PCR and nested PCR. The patients were divided into two groups, based on the presence of API2-MALT1 fusion or not. It was done to compare the clinicopathological features and survival between the two groups. Results API2-MALT1 transcripts were detected in 11 of 32 cases (34.4%). API2-MALT1-positive patients showed earlier stage, lower frequency of involvement of lymph node and proliferative index than those of API2-MALT1-negative. The follow-uq data indicated that API2-MALT1-positive patients presented lower relapse and higher fiveyear survival than API2-MALT1-negative ones. Conclusion There was obvious correlation between the expression of API2-MALT1 mRNA and clinicopathological features and prognosis of extranodal DLBCL. The cases of DLBCL from the transformation of MALT lymphoma showed more indolent clinical course and better survivial. The morphological transformation from MALT lymphoma to DLBCL could imply the progression of this spectrum.
