耐药基因蛋白P-gp、Topo-Ⅱ在原发型肝细胞癌中的表达及意义

摘　　要：目的　探讨原发型肝细胞癌中耐药基因蛋白P-gp、Topo-Ⅱ的表达与原发型肝细胞癌病理特征以及生物学特性之间关系。方法　应用免疫组织化学方法检测37例原发型肝细胞癌组织中P-gp、Topo-Ⅱ的表达,分析其与病理特征及肿瘤生物学特性之间的关系。结果　P-gp的表达与AFP的增高,HBV的感染以及肝癌的远处转移有关,与年龄、肿瘤大小、病理分级、临床分期、门静脉癌栓形成、肝内转移以及转氨酶变化均无关。Topo-Ⅱ的表达与肝内转移有关,与年龄、肿瘤大小、病理分级、HBV感染、门静脉癌栓形成、AFP值、远处转移及转氨酶高低之间无关。P-gp的高表达与Topo-Ⅱ的低表达相关。结论　临床检测肝癌耐药基因蛋白P-gp、Topo-Ⅱ的表达不仅可以作为原发型肝细胞癌化疗方案选择的参考,也可以用于预测肿瘤发展。Objective To study the expression of P-gp and Topo-Ⅱ in hepatocellular carcinoma (HCC) and to explore the relationship between the expr ession of P-gp and Topo-Ⅱand pathological characteristics and clinical signific ance. Methods Thirty seven histologically verified HCC s pecimens were examined for P-gp and Topo-Ⅱby immunohistochemical methods. Results The expression of P-gp was related to AFP, HBV infecti on and distant metastasis. Its expression was not related to age of patients, tu mor size, histological grade, local metastasis and so on. The expression of Topo -Ⅱwas related to local metastasis. Its expression was not related to age of pat ients, tumor size, histological grade, distant metastasis , AFP ,HBV infection a nd so on. Conclusion The expression of P-gp and Topo- in hepatocellular carcinoma played an important role in chemotherapy strategy and m etastasis of HCC.
