摘 要:目的 探索瓦斯爆炸伤致伤心肌缺氧后HIF 1αmRNA及其蛋白表达的动态变化规律及其在瓦斯爆炸伤心肌继发性损害发病机制中的作用。方法 建立瓦斯爆炸伤动物实验模型,并分析爆炸后3 0min、2、4、8、12、2 4、48h时段心肌组织大体、光镜及超微结构特点;用地高辛标记HIF 1αcDNA探针进行心肌组织原位杂交;用RT PCR方法对心肌组织HIF 1αmRNA进行半定量检测,用S P法进行心肌组织HIF 1α及wtP5 3免疫组化染色。结果 存活动物全身表皮可见不同程度烧伤,以四肢为重,实质脏器未见明显破裂,心内膜下可见附壁血栓。电镜下见:①心肌细胞水肿,线粒体肿胀、空化,内质网扩张;②部分肌丝排列紊乱,肌丝有局灶性溶解,闰盘分离;③核形不规则,异染色质边集,可见核溶解;④毛细血管周围间隙增宽,内皮细胞肿胀、变性,管腔狭窄。此病变在4h内明显,随时间延长,病变程度减轻。HIF 1αmRNA及其蛋白表达明显增加,且2h达高峰值,明显高于3 0min及48h组,对照组无表达(P <0 0 1)。wtP5 3表达明显增加,2h为高峰值。结论 HIF 1α在瓦斯爆炸伤致心肌缺氧的继发性损害发病机制中起重要作用,且与损伤程度密切相关,wtP5 3可降低HIF 1α的活性,减少HIF 1α的过量表达,增加wtP5 3的活性及量可减轻瓦斯爆炸所致的心肌损伤,对心肌起保护作用。Objective To explore the dynamic changes of HIF-1α mRNA and its production and roles in the mechanisms of secondary myocardial lesions caused by gas explosion. Methods Animal model of gas explosion injury was established. The pathological features in myocardial tissues were observed macroscopically and light microscopically at 30 min, 2, 4, 8, 12, 24, and 48 h after explosion. Meanwhile, HIF-1α was determined by in situ hybridization. Semi-quantitative RT-PCR was used to measure HIF-1α mRNA. HIF-1 and wtP53 in the myocardial tissues were measured by immunohistochemistry. Results Different degrees of burn of the epidermis, especially that of the limbs, of the survived animals were found. There was no obvious rupture of the organs. Thrombi were observed in the ventricle. There were ultrastructural alterations in the myocardial tissues. The expression of HIF-1α mRNA and protein increased obviously after explosion and peaked at 2 h. The expression of wtP53 increased obviously after explosion and peaked at 2 h. Conclusion HIF-1α plays an important role in myocardial ischemia in rats and is closely associated with injury severity caused by gas explosion. wtP53 can decrease the activity and over-expression of HIF-1α, so enhanced activity and expression of wtP53 may alleviate the damages to myocardial tissues.
