摘  要:目的 探讨离体脑膜瘤和胶质瘤魔角旋转高分辨质子波谱(HRMAS1H MRS)特点及其在病理分级中的价值。资料与方法 经手术病理证实的脑肿瘤32例(脑胶质瘤2 1例,脑膜瘤11例) ,其中胶质瘤又分为良性组、过渡组和恶性组。将其离体标本在室温下6 0 0MHz波谱仪上行HRMAS1H MRS采集。检测的化合物包括氮乙酰天门冬氨酸(NAA) ,胆碱类化合物(Cho)、谷氨酸和谷氨酰氨(Glu Gln)、甘氨酸(Gly)、牛磺酸(Tau)、肌醇(mI)、丙氨酸(Ala)、乳酸(Lac)等。将3组胶质瘤代谢物相对浓度及脑膜瘤和胶质瘤的代谢物相对浓度进行比较。结果 (1)Cho/Cr在良性组与过渡组和良性组与恶性组间有显著性差异(P <0 .0 5和P <0 .0 0 1) ,而过渡组与恶性组间无显著性差异(P >0 .0 5 ) ;Gly/Cr在良性组与恶性组间存在显著性差异(P <0 .0 5 ) ,良性组与过渡组及过渡组与恶性组间无显著性差异(P >0 .0 5和P >0 .0 5 ) ;Tau/Cr在良性组与恶性组和过渡组与恶性组间有显著性差异(P <0 .0 5和P <0 .0 5 ) ,而良性组与过渡组之间无显著性差异(P >0 .0 5 ) ;NAA/Cr、Ala/Cr、mI/Cr、Glu Gln/Cr、Lac/Cr和Lip/Cr组间差异无显著性(P >0 .0 5 )。(2 )在胶质瘤和脑膜瘤的比较中,脑膜瘤中的Ala/Cr、Cho/Cr、Glu Gln/Cr、Tau/Cr和Lac/Cr都较胶质瘤高,其差异有?Objective To evaluate the value of in vitro High-Resolution Hydrogen Magic Angle Spinning magnetic resonance spectroscopy (HRMAS 1H-MRS) in the differentiation from gliomas to meningiomas and characterizing their spectral patterns guiding to grade glial tumors.Materials and Methods 21 gliomas and 11 meningiomas were confirmed by surgical pathology. Among of them, 21 gliomas were divided into three groups: benign group (n=10), transitive group (n=5) and malignant group (n=6), which were analyzed by HRMAS 1H-MRS. The HRMAS 1H-MRS measurement was performed on a 600 mHz NMR spectrometer at room temperature. On the basis of these HRMAS 1H-MRS spectra, we measured the concentration of 10 metabolites, including NAA, PCr/Cr, Cho, Glu/Gln, Gly, Tau, mI, Ala, Lac, Lip and the ratios of 9 metabolites to Cr.Results (1)By the compare of three gliomas groups, the Cho/Cr ratios was existed significant difference from benign group to transitive group and from benign group to malignant group (P<0.05 and P<0.001), but there was no significant difference between transitive group and malignant group (P>0.05). The Tau/Cr ratio also was existed significant difference from malignant group to benign group and from malignant group to transitive group (P<0.05 and P<0.05), but there was no significant difference between benign group and transitive group (P>0.05). The Gly/Cr ratio was existed only the significant difference between benign group and malignant group (P<0.05). The ratios of NAA/Cr, Ala/Cr, mI/Cr, Glu-Gln/Cr, Lac/Cr and Lip/Cr were existed no significant difference among the three groups (P<0.05). (2)Between gliomas group and meningiomas group, the ratios of Cho/Cr, NAA/Cr, Ala/Cr, Tau/Cr, Glu-Gln/Cr, Lac/Cr were existed significant difference (P<0.05), the ratios of mI/Cr, Gly/Cr were existed no significant difference (P>0.05).Conclusion HRMAS 1H-MRS can provide plentiful biochemical data of gliomas and meningiomas, which exists unique value in the differentiation from gliomas to meningiomas and evaluation of glial tumor g
