乳腺浸润性导管癌钼靶X线表现与雌激素受体、孕激素受体及人类上皮生长因子受体-2的关系

摘　　要：目的　探讨浸润性导管癌(IDC)钼靶X线表现和雌激素受体(ER)、孕激素受体(PR)及人类上皮生长因子2之间(c -erbB- 2)的关系,评价导管癌钼靶X线表现,术前预测浸润性导管癌的ER、PR、c- erbB- 2的表达的可行性。方法　35例乳腺钼靶和临床病理证实的浸润性导管癌,术后大体标本经免疫组织化学染色判断这些免疫因子的表达情况。把浸润性导管癌钼靶X线表现中的恶性钙化、肿块边缘的毛刺与ER、PR及c -erB- 2表达情况进行对比。结果　35例导管癌中, 肿块伴毛刺者25例,有钙化者14例;ER阳性者21例,毛刺组ER阳性率与无毛刺组相比有显著性差异(P<0 .05)。无钙化组ER阳性率较有钙化组高,但无统计学意义;PR表达阳性者23例,毛刺组PR阳性率较无毛刺组高,钙化组PR阳性率较无钙化组略高,无统计学意义; c- erbB- 2表达阳性者25例,无毛刺组c -erbB -2阳性率较毛刺组高,无统计学意义。钙化组c -erbB -2阳性率与无钙化组相比有显著性差异(P<0. 05 )。结论　IDC中毛刺性肿块与ER表达、钙化性肿块和c- erB- 2、PR表达之间有相关性,钼靶X线征像可以粗略反映ER、PR及c- erB -2表达情况。Objective To probe the relationship between the appearance on mammography and estrogen receptor (ER), progesterone receptor (PR) and c-erbB-2 in infiltrating ductal carcinoma of breast (IDC) and evaluate the possibility of predicting the expression of ER,PR and c-erbB-2 in IDC pre-operation by means of mammography. Methods 35 patients with IDC confirmed pathologically were involved in this study, and the specimen was examined immunohistochemistically to judge the expression of the related factors. The malignant calcification,marginal speculation around tumors on Mo-target X-ray images were compared with the expression of ER, PR and c-erbB-2. Results Of the 35 cases, spicule sign was demonstrated in 25, and calcification was seen in 14,while ER positive expression was seen in 21. Significant difference in ER positive rate existed between lesions with spicule sign and lesions without spicule sign( P< 0.05). ER positive rate in lesions without calcifications was higher than that in lesions with calcifications, although no statistically significant difference was found. PR positive expression was seen in 23,PR positive rate in lesions with spicule sign was higher than that in lesions without spicule sign. PR positive rate in lesions without calcifications was slightly higher than that in lesions with calcifications, but no statistically significant difference was found. c-erbB-2 positive expression was seen in 25. c-erbB-2 positive rate in lesions without spicule sign was higher than that in lesions with spicule sign, although no statistically significant difference. Significant difference in c-erbB-2 positive rate existed between lesions with calcifications and without calcifications ( P< 0.05). Conclusion A certain correlation exists between spicule sign and ER expression,PR expression and c-erbB-2 expression in patients with IDC. The appearance of infiltrating ductal carcinoma (IDC) on Mo-target X-ray images may be used to predict the expression of ER,PR and c-erbB-2 in IDC.
