一氧化氮合酶在恶性黑素瘤皮损中的表达

摘　　要：目的　研究神经型一氧化氮合酶(nNOS)、诱导型一氧化氮合酶(iNOS)和内皮型一氧化氮合酶(eNOS)在皮肤恶性黑素瘤与色素痣中的表达,并探讨其意义。方法　应用免疫组化方法检测4 3例恶性黑素瘤nNOS、iNOS和eNOS的表达。结果　①nNOS在黑素瘤细胞中表达率6 7.4 4 % ,在色素痣细胞中表达率2 0 % ,其中在原发无转移的黑素瘤中,阳性表达17例,阴性表达9例;转移的黑素瘤中,阳性表达12例,阴性表达5例。②iNOS在黑素瘤细胞中表达率76 .74 % ,在色素痣细胞中表达率90 % ,其中在原发无转移的黑素瘤中,阳性表达16例,阴性表达10例;有转移的黑素瘤中,阳性表达17例,无阴性表达。③eNOS在黑素瘤细胞中表达率72 .0 9% ,在色素痣细胞中表达率80 %。其中原发无转移的黑素瘤中,阳性表达15例,阴性表达11例;有转移的黑素瘤中,阳性表达16例,阴性表达1例。结论　①nNOS在黑素瘤细胞与色素痣细胞中的阳性表达率具有显著差异,可作为鉴别良恶性黑素细胞肿瘤的指标之一。②iNOS和eNOS在恶性黑素瘤转移中起促进作用。Objective To observe the expression of nNOS,iNOS and eNOS in malignant melanoma and nevus cell nevus and to discuss its significance.Methods Immunohistochemical methods was used to examine the expression of nNOS,iNOS,eNOS in 43 patients with malignant melanoma and 10 patients with nevus cell nevus.Results ①The positive expression of nNOS was observed in 67.44% malignant melanoma and in 20% nevus cell nevus.Positive was detected in 17 patients with non-metastatic melanoma and negative in 9 patients.Positive was detected in 12 patients with metastatic melanoma and negative in 5 patients.②The positive expression of iNOS was observed in 76.74% malignant melanoma and in 90% nevus cell nevus.Positive was detected in 16 patients with non-metastatic melanoma and negative in 10 patients.Positive was detected in 17 patients with metastatic melanoma and no negative.③The positive expression of eNOS was observed in 72.09% malignant melanoma and in 80% nevus cell nevus.Positive was detected in 15 patients with non-metastatic melanoma and negative in 11 patients.Positive was detected in 16 patients with metastatic melanoma and negative in 1 patient.Conclusion ①There is significant difference of nNOS expression between patients with malignant melanoma and nevus cell nevus.It can be used as sign to discriminate between benign and malignant tumor.②iNOS and nNOS plays a positive role in malignant melanoma development and metastasis.
