摘  要:目的对22例脊髓多发性硬化 MRI 资料进行回顾性分析,对其诊断及鉴别诊断进行探讨。方法所有病例均为临床(Poser 分类)及实验室检查所证实。对脊髓 MS 病灶按部位、信号强度及形态特点分别进行分析。结果:颈段脊髓为脊髓 MS的好发部位,其部位特点与该病临床症状有很强的相关性。脊髓 MS 病灶分为斑块灶和弥散信号异常区两类,前者 T1WI 呈等信号,T2WI 呈高信号,边界清楚,见于慢性期及少部分急性期病例。后者以 T1WI 等信号为主,少数呈低信号;T2WI 均呈较高信号,边界模糊不清,均见于急性期。急性活动期脊髓肿胀,而慢性期脊髓萎缩。平扫矢状位 T2WI 对病灶的显示最敏感。脊髓 MS 与脊髓感染性炎症和脊髓缺血梗死的 MRI 鉴别诊断有一定难度。结论 MRI 对脊髓 MS 的定位、定性及分期具有很高的价值。Objective A MRI retrospective analysis of 22 cases of multiple sclerosis(MS)in spinal cord has been done to determine its value in diagnosing and differentiating from other inflammatory disorders and cerebrovascular diseases.Method All of the 22 MS cases were either clinically defined under Poser criteria or laboratory supported.We assess the location,signal intensity and lesion characteristics of MRI in spinal cord MS.Results Spinal cord MS is mainly found in cervical spine and it has a well correlation with its symptoms.MS spinal cord abnormalities comprise two types.The first type,focal lesions,which consists of well-demarcated areas of iso-intensity on T1WI and high signal intensity on T_2WI,are seen mainly at chronic stage and few of acute stage of spinal cord MS.The second is diffuse abnormalities,which can only be found in acute MS cases,comprise poorly demarcated areas of iso-signal intensity on T1WI and comparatively increased signal intensity on T2WI.Con- clusion Spinal cord MRI has a high value in diagnosing spinal cord MS.
