摘  要:目的探讨女性超重、肥胖者脑血管血液动力学(CVH D)变化特征及其与高血压、糖尿病、心脏病等相关疾病的关系。方法选择2001年进入卒中队列研究人群的1336例35~65岁女性为研究对象,进行卒中危险因素等基线调查和CVH D检测并积分。将研究对象按体质指数(BM I)分为体重正常组(BM I<24.0kg/m2)、超重组(BM I为24.0~27.9kg/m2)和肥胖组(BM I≥28.0kg/m2),比较3组间CVH D及其积分的差异。结果上述3组CVH D积分均值分别为(92.51±15.93)、(81.83±23.71)和(73.32±27.09)分(P<0.001);CVH D指标中脑血流量、血流速度依次降低(P<0.01),脉搏波波速、特性阻抗、外周阻力、动态阻力依次升高(P<0.01);3组对象中高血压患病率分别为19.28%,37.60%和38.92%(P<0.01),心脏病患病率分别为14.62%,21.26%和21.62%(P<0.01),糖尿病患病率分别为4.04%,4.53%和4.86%(P>0.05)。结论女性随着BM I的增大,CVH D积分水平明显降低,脑血流量减少,血流速度减慢,血管弹性变差,且高血压、心脏病、糖尿病等相关疾病的患病率升高。Objective To discuss the characteristics of cerebral vascular hemodynamics CVHD in obesity and overweight women and its relations to hypertension diabetes and cardiopathy. Methods 1 336 women aged 35 to 65 years who participated in a stroke cohort study in 2001 were selected. During baseline investigation stroke risk factors such as hypertension diabetes cardiopathy etc. were investigated CVHD indexes were examined and accumulative score of CVHD was estimated. Participants were divided into three groups such as normal group BMI<24.0 kg/m2 overweight group(BMI24.0-27.9)and obesity group BMI≥28.0. CVHD as well as its accumulative score were compared between groups. Results The CVHD scores in three groups were 92.51±15.93 81.83±23.71 and 73.32±27.09 P<0.001 respectively. Among CVHD indexes Qmean Vmean Vmax and Vmin decreased in file P<0.01 WV Zcv RV DR and CP increased in file P<0.01. The prevalence rates of hypertension in three groups were 19.28% 37.60% and 38.92% P<0.01 cardiopathy were 14.62% 21.26% and 21.62% P<0.01 diabetes were 4.04% 4.53% and 4.86%(P>0.05) respectively. Conclusions With BMI increasing CVHD score was significantly decreased quantity and velocity of cerebral blood flow was decreased flexibility of cerebral vessel became worse and these were also related to the increase of prevalence rate of hypertension diabetes and cardiopathy in women.
