摘　　要：目的　探讨血管内皮生长因子(VEGF)、基质金属蛋白酶- 9 (MMP- 9)及E 钙粘素在甲状腺癌组织中的表达情况与甲状腺癌的组织类型、临床分期和淋巴结转移的关系。方法　采用免疫组化SP法检测50例甲状腺癌患者癌组织标本及20例甲状腺癌患者癌旁正常甲状腺组织中VEGF、MMP -9及E 钙粘素的表达情况,观察其与患者临床病理及预后的关系。结果　甲状腺癌组织中VEGF的表达显著高于癌旁组织,且与甲状腺癌的组织类型、临床分期和淋巴结转移密切相关;MMP- 9的表达亦显著高于癌旁组织,与甲状腺癌的组织类型无关,但与临床分期和淋巴结转移密切相关。分化型甲状腺癌组织中E 钙粘素的表达显著低于癌旁组织,与甲状腺癌的组织类型无关;未分化癌中E- 钙粘素呈阴性表达,E -钙粘素的表达与临床分期和淋巴结转移呈负相关。结论　甲状腺癌组织VEGF、MMP- 9及E 钙粘素的表达与甲状腺癌的侵袭及转移有关,可作为甲状腺癌患者的预后指标。Objective To investigate the correlation between the expression of vascular endothelial growth factor (VEGF),matrix metalloproteinase(MMP-9),E-cadherin and histological feature,clinical stage,lymph node metastasis in thyroid carcinoma.Methods The SP immunohistochemical method was employed to determine the expression of VEGF,MMP-9,E-cadherin in 50 thyroid carcinoma and 20 para-tumorous tissue.Results The expression of VEGF in thyroid carcinoma was remarkably higher than that in the para-tumorous tissue,and significantly associated with histological feature,clinical stage and lymph node metastasis.The expression of MMP-9 in thyroid carcinoma was also remarkably higher than that in the para-tumorous tissue,which was uncorrelated with histological feature,but correlated with clinical stage and lymph node metastasis.The expression of E-cadherin in differentiated thyroid carcinoma was much lower than that in the para-tumorous tissue,which was uncorrelated with histological feature.The expression of E-cadherin in differentiated thyroid carcinoma was negative,which had an negative correlation with clinical stage and lymph node metastasis.Conclusion The expression of VEGF,MMP-9,E-cadherin is associated with invasion and metastasis of thyroid carcinoma,which may serve as prognosis indexes of thyroid carcinoma.
